You can anticipate a 2 inch incision under the chin in the submental crease. You can also anticipate incisions hidden along the sideburn, the front of the ear, and down around the ear into the back of th neck. hairline. All of these incisions are so well hidden after they heal that they should not be noticeable after 1 year. Greetings, and thanks for the question. This is a common one we get in clinic. The majority of facelifts have incisions that are designed to provide access to the areas at and below the ears down to the area of the adams apple. Modern incisions are designed to blend into natural transition points/skin tension lines. I typically place them at the base of the hairline in front of the ear, then blending into the transitional zone along the front of the ear and ear canal before wrapping around the earlobe and forming and upside down U hugging the back of the ear until following the hairline behind the ear. By putting the incision at the border of all the hairlines, skin can be removed as much as necessary without creating a hairline "step-off" anywhere. This incision provides access to the skin and muscle that will be resuspended to eliminate jowels and the lateral loose neck. The other incision is placed in the wrinkle just below the chin. This incision provides access to tighten the loose central neck. Just as important as the location of the incision however is the type of facelift. Skin heals best when there is no tension on the incision and this is achieved with a deep plane facelift. The skin is left connected to the underlying muscle, which does the actual holding. With optimal placement and a deep plane approach, the incisions are usually not noticeable after 2-3 months.
I will recommend having a consultation with a board certified plastic surgeon so the incision can be shown more clear as well how they heal; The most part of the techniques for facelift require an incision beneath the chin fold as well around the ears ( front and back) following the natural contouring and folds of them so this are hidden and barely noticeble. Incisions in front and behind the ear trailing into the hairline behind the ear and under the chin are the standard locations for a face lift.Less incisions may be used but I think the result is compromised. Good Luck!
The incisions surgeons use for facelifts/ necklifts vary from one surgeon to the next. I would recommend asking the surgeon you meet in consultation this question so they can show you themselves.Personally, I would make an incision as imperceptible as possible by hiding it in the hair and around your ears:-The temporal portion is in the hair (as opposed to along the hairline border which can show with time)-The pre-auricular portion goes inside the ear behind the tragus and where your ear helix meets your cheek as well as where the earlobe meets your cheek-The post-auricular incision hides in the crease just behind your ear where the ear attaches to your scalp and extends directly back into your hair to hide it (as opposed to along the nape hairline which can show)The neck incision is a small horizontal incision just underneath your chin where many people have scars from childhood (chin hitting a coffee table, etc) and hides very easily.I recommend looking up some photos/ diagrams as a picture is worth 1000 words!Best of luck!

Thank you for your photographs. You have skin laxity of your cheek, jawline and neck. You also have some redundant platysma muscle in the midline of your neck. In order to fully correct this, you would require an incision which is just in the hairline above your ear, extending down along the natural contours on the front of your ear, up behind your ear and then again down along the hairline partially down your neck. Redundant platysma muscle, in the midline under your chin, may need to be contoured and in which case a 3 cm incision under your chin allows the best access to that. This would allow complete visualization of all of the areas which would require correction to fully rejuvenate your appearance. This would all be explained in detail at the time of your consultation. There is a natural vector that is followed with most deep plane facelifts that is more diagonal than vertical. It is possible to address the cheek or malar area with adding back volume via fat transfer. In that way you have the benefit of both the lifting of the SMAS and cheek.
