At 5'8 and 285 lbs, your BMI is 43 which puts you at high risk for complications. You can undergo liposuction. Liposuction can safely remove 10 lbs during one session and the sessions can be repeated if needed. Once you get your BMI down to 32 or below, your can consider a mommy makeover. I would begin with your breasts using The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained. After this has been done, you can consider a tummy tuck. If you are not planning future pregnancies, I recommend abdominoplasty with mesh reinforcement. The mesh creates a more hourglass figure by compressing the abdominal wall resulting in early satiety and helps maintain a reduced weight.

generally speaking, you want to reach your goal weight before you consider a tummy tuck, otherwise your results will be suboptimal if you continue to lose weight after the surgery.If you are considering surgery, I would suggest you consult a board-certified plastic surgeon. I would suggest going with liposuction for now to remove excess fat of your abdomen and flanks. Also, weight loss would be very helpful to get you into a much healthier weight, and no fat transfers should be done during weight loss. A weight of 285 in a women of 5'8" is not "good health."

If you are happy with your figure and current weight, then you are good candidate for a plus size tummy tuck and breast augmentation with fat transfer. From you photos, I cannot tell if you will also need a breast lift. Best is to consult with a board certified plastic surgeon that is comfortable with performing a plus size abdominoplasty.

You will need to consider weight loss before having a Mommy Makeover. Those with a high BMI have higher risks from surgery including seroma, wound healing issues, and infection. Losing weight first will also allow your surgeon the opportunity to maximize your results.

While your skin looks good in photos, your weight is too high for an elective procedure (by my standards). I perform procedures on very large healthy patients with a BMI up to 39 (BMI-Body Mass Index). Your BMI is 43.3 which is too high for me to consider. If you lose weight (255-260 pound goal), you might be considered for an elective procedure.
You might be able to do liposuction, but a physical exam determine if an abdominoplasty is indicated.

The risks of severe complications in your case make surgery a dangerous choice. In fact if a surgeon says yes to you, run away.

Now that I have shocked you with my truthful comment, don't despair. Bariatric surgery could be your salvation. There are several options. There are medications that work well too.

After losing weight to a safe level, your surgical results will amaze you. Your target weight is 190 ish and a typical result of gastric sleeve or bypass.

Thanks for questions & photos. The surgery of TT with lipo and fat to breasts can be done. In Florida office surgery limited to 1 liter lipo so will need a second surgery for more lipo . That fee $13,000. If out patient hospital can do more lipo with TT and fat transfer fees $18,000+. Best to virtual consult….

If you want us to make an assessment for breast surgery then you need to show your breast.

At your current weight you’re not a good candidate for any cosmetic surgical procedure so start working on weight loss first. After successful massive weight loss(100 lbs) you’ll be an excellent candidate for an extended or wraparound tummy tuck and breast lift with implants. Fed transfer is probably not going to be a good option for you. In the end it depends what you’re trying to achieve but most likely you want your breast to have a better shape and fat transfer doesn’t do that.

To get projection you need implants and to get your nipple to sit in the right position requires a lift.

You can begin consulting with plastic surgeons now if you want but you are not a good candidate at your current weight.

With weight loss you could potentially be an excellent candidate for these procedures.

From your photos, I note a full abdomen, flanks, and hips. You demonstrate a hanging abdominal pannus and moderate stretch marks extending to a level above the umbilicus.
BMI, or body mass index (weight in kg / square of height in meters) is one criterion to determine the results of a cosmetic surgery. You can find BMI calculators on the internet. If your BMI is greater than 30, the cosmetic result will not be optimal. If your BMI is greater than 35, your operative risk will rise. Your BMI is 43.33.
Based upon this information, one should consider a program of weight loss before embarking upon cosmetic surgery. Your surgical risk is significant at this BMI. A mommy makeover is major surgery.
If diet and exercise fail to drop your BMI to a safe value, you may consider weight loss surgery. The gastric sleeve procedure, performed by a bariatric surgeon, will result in a significant amount of weight loss. It is now being performed on an outpatient basis!
