This is an urgent issue, and you should go to the emergency room at a large hospital.  They will be able to refer you to a specialist that can help you manage this (which means surgically removing the implant).  Perhaps, you should call the ER and ask them if they have plastic surgery coverage.  Good luck.

 This needs to be removed and the skin edges closed. See a very experienced revision rhinoplasty surgeon for this along with later reconstruction using your own septal cartilage.

 You need to see a physician to have the implant removed  due to the fact that the area is almost certainly infected. You will probably need to then wait for several months to have repeat surgery to reconstruct the nose. You will likely need cartilage from another source, such as the rib or the ear.

Revision rhinoplasty is a surgery to improve the shape of the nose or make corrective changes for better breathing or appearance. It is also a means to repair/reconstruct a complication, such as an infected or extruded implant.Although most patients do well with implants, the unfortunate few who have infection and extrusion need aggressive management. If it is a minor infection, you can be treated with antibiotics in preparation for a reconstructive surgery where the implant is removed and replaced with rib cartilage. If it is a major infection, you'll need two surgeries. One to remove the implant and clean dead tissue. A second surgery to reconstruct the defect with rib cartilage. Antibiotics needs to be started right away. I would consult with a surgeon who has seen this type of problem before as it is a complex issue. Safety comes first.

I recommend immediate evaluation by a physician. You should go to your nearest emergency department to be evaluated by a ENT (ear, nose and throat surgeon) or plastic surgeon. You will need this removed under anesthesia and you will likely require antibiotics postoperatively and revision surgery. Reconstructive surgery with your own cartilage (rib or ear) will be safest option going further. Best of luck!

You require immediate medical/ surgical attention for your infection and extruded silicone implant. Go immediately to your local hospital emergency room and you will be referred to a specialist who can help you. This implant need to come out under anesthesia and any infection debrided/ irrigated. You may require additional future surgery for reconstruction or scar. 

Lucky for you, now there is organ donated rib cartilage that you will need to have your nose revised. This will save you from having to harvest your own cartilage. Please make an appointment with a board certified surgeon who has experience with using rib cartilage grafts.

You need to have the implant removed.  After several months, we can reconstruct with your own cartilage.  We have take care of patients like this before and have seen good results once it is all healed.  Best wishes.
