I recently had a facelift (neck, mid face and brow) and am very happy. I also had dermabrasion around the mouth. But I still have lines and wrinkles above and around the mouth that detract from the overall effect. I've been treated with Radiesse twice for them, but then other lines seem to form. What's my best solution for getting rid of lines and wrinkles around the mouth more permanently?
Answer: Wrinkles around the mouth following facelift I would recommend some type of resurfacing procedure such as C02 laser. It is very safe and is considered the gold standard for improving wrinkles. Fat grafting may help significantly since often wrinkles are a result of volume loss as well. If your prefer to use other injectables, I would recommend any number of the hyaluronic acid products currently available.

Answer: For deep wrinkles and line around the mouth it is recommended to do CO2. Applying a small amount of filler can also help Fractional laser is an amazing and highly effective treatment in our office. We use the laser treatment to help with acne scars, large pores, fine wrinkles, tissue collagen loss, scars, sebaceous hyperplasia, active acne, and for brightening dark pigmentation. Although there are very strong fractional lasers such as CO2 laser, we use lighter treatments for corrective skincare and maintenance of results. Our Thulium fractional laser, clear and brilliant laser, and Ultra Thulium laser are excellent options in our clinic. We also perform CO2 fractional and erbium depending on skin tone and tolerance for downtime. Patients do have to use a Melarase cream before and after treatment to help further reduce pigmentation after laser. Melarase AM, Melarase PM, and Melapads can seriously improve surface sun damage and UV related pigmentation. Dark spots can improve with these treatments. For the neck we need to tighten the platysma and do a submentoplasty (under the chin incision). The best thing to do is find a surgeon you trust and go over your goals slowly and clearly. Dermabrasion is difficult to treat these lines as well unless it is of sufficient depth. In my practice, for moderate to severe smile lines, the Erbium laser works well. This is an ablative laser which means it removes the outer layers of your skin using controlled heat. This not only softens these wrinkles but also aids in collagen production which provides a more youthful appearance. People with lighter skin tones are better candidates for this laser as darker pigmented individuals are more prone to developing hyperpigmentation. It is very important to consult with a board certified plastic surgery with extensive experience and expertise in laser surgery.

Answer: Solutions for Wrinkles around the Mouth Wrinkles around the mouth are called Marionnette lines (lateral to oral commissure) and Vertical Rhytids around the central lip areas. These are very difficult areas to fix as a Facelift only pulls this aea laterally and the dissection does not and cannot extend into the lip area. Every Facelift I perform gets a Dermabrasion treatment that involves diamond burr sanding of the peri-oral area to the interface between deep and superficial dermis to stimulate new collagen production. This remains the Gold Standard for treatment of these lines but may need to be repeated 2-3 times. It is not possible to completely get rid of the lines and any treatment that promises this is not true.
