Follow-up: 18-year-old, size DD, saggy and wide apart breasts, what's the best surgery in my case? (Photos)

I am the 18 year old with the saggy and very wide apart breast. I have back & neck pain, my boobs make me immensely insecure to the point where I don't get naked in front of any one, they look horrible to me and I get so sad every time I look at them. Which surgery would best benefit me? The overall results I'm looking for are much more lifted breast (no sagging), and a very lessened boob gap, as much as possible.
