The thing that bothers me most about my nose is my ill-defined tip. I had a consultation with a doctor who said tip plasty alone would be feasible. During my research, I encountered this article: Interdomal Suture through a Nondelivery Endonasal Approach: A New Technique by Harel and Leibou It made me wonder: would it be possible to have interdomal sutures places under local anesthesia to make a minor, less invasive change to my wide nasal tip? Could this be done as a stand-alone procedure?
Answer: Tip plasty for select patients who are not anxious and can handle local anesthesia procedures, we have done tip-plashy nose work (and more) in our offices without problems. Be sure to discuss your goals and concerns with your board certified plastic surgeon and consider the pros and the cons. Be certain your surgeon is properly certified. It is extremely difficult to give specific advice about nasal tip surgery without evaluating your tip. Make sure you specifically look at before and after pictures of real patients who have had this surgery performed by your surgeon and not just a computer animation system. The most important aspect is to find a surgeon you are comfortable with. I recommend that you seek consultation with a qualified board-certified plastic surgeon who can evaluate you in person. Answer: Tip rhinoplasty Tip work represents some of the most intricate portions of the rhinoplasty surgery. As such, it is not simple and should be done under the most ideal circumstances. I would advise against attempting it under local anesthesia.

I feel that your first concern should be an excellent surgical outcome that you are confident with and one that will last and you will enjoy for many years. Choosing a surgeon for your rhinoplasty procedure based on the promise of "minimally invasive" rhinoplasty and minimal downtime does not guarantee that your goals will be met. As the patient you have complete control (within reason) over how you would like your nose to look. The surgeon should perform the appropriate surgery based on their experience in order to deliver you the nose you desire.
