40 years old. Gained 32lb then lost 20, will lose 15 more. My huge cheeks sagged down with the weight loss (yak). Now i have a horrible turkey neck, and intolerable jowling. You can see that my face is very heavy in general (front view pictures). The turkey neck and jowls are obvious in the magnified side view pictures. I want a perfect 90° neck/chin angle, not just a little improvement. What are my options. Neck lift, mini neck lift, neck Lipo, chin implant, or a combination of a few? Thanks.

Answer: Chin Implant, Neck Lipo, Neck Lift Most importantly: congratulations on your weight loss. It is excellent that you are reversing your weight gain, and protecting your health. To create a sharper jawline and eliminate the turkey neck and jowling, I would perform a neck lift with all incisions hidden behind your ear crease. I would insert an intraoral chin implant (I insert the implant through an incision in your gums, thereby not creating any external scars and preventing the implant from potentially slipping in the future, as it can when the incision is on the external skin of the chin). I would also very importantly liposuction your neck to remove excess fat. The proportions of all of these elements -- how much skin to resect, what size implant to use, and how much fat to remove -- are all the nuanced elements of a plastic-surgeon's aesthetic. When done with sophistication, the results can be natural-looking and spectacular. Again, keep up the good work with your weight loss.

You need to realize that no surgeon can guarantee the 90 degree neck that you are looking to have, so be realistic on what can be achieved. The results will depend on your anatomy. Currently you have a heavy neck and face. On your physical exam the surgeon has to determine the position of your hyoid bone. The more anterior and lower it is, the less likely it would be to get that 90 degree neck. In your case (only looking at the pictures) it appears that your hyoid bone is in an intermediate position. This means that you might be able to get a good decent result, but most probably not the perfect one that you are looking for.I would recommend that you consider a neck lift with liposuction and a chin implant.
