After my platysmaplasty, I still have vertical lines down my neck. They appear to be the same ones, or new ones next to the old.

Recurrence of vertical lines down the neck (platysma bands) after a platysmaplasty could occur in some patients. Recurrence or appearance of new platysma bands could be improved temporarily and non-surgically with Botox application. More definitive long term management requires re-operation with revision platysmaplasty. Sometimes, in very selected patients, neck bands could be addressed with percutaneous platysmotomy, a minimally invasive procedure that addressed the neck bands

It is rare for patients to have neck lines reappear after receiving platysmaplasty. I would point you towards more long-term solutions. Vertical bands in the central neck are from the inner edge of the platysma muscle which has loosened over time because of weakening of the cervical fascial attachments. The platysma should be brought together with a durable repair in a corset type fashion with a lower platysmal myotomy to help avoid recurrent banding. This central platyma repair with an outer neck SMAS lower facelift should help to eliminate banding and improve the contour in the neck. If banding is present after a necklift procedure, either the repair is disrupted or it was not performed in the primary procedure.

A platysmaplasty is a surgical procedure that
creates angulation of the neckline below the chin from the profile by tying the
right and left platysma muscles together in the middle.  This is a common technique used during a neck lift. The platysma muscle is a
long thin muscle that goes from the jawbone to the clavicle. The muscle do not
meet in the middle. During a plastysmaplasty, these muscle are stitched
together to tighten the neck. These loose muscle edges are causing the vertical
bands in the middle of the neck.There are times after a platysmaplasty where the neck lines or bands can return. The overwhelming cause of the recurrence of these bands are the reappearance of the old bands. If this happened in your surgery, the bands can be re-stitched together to improve the area. Unfortunately, these bands can return after a platysmaplasty.I hope this helps

lorimkeep-thanks for asking this question. Unfortunately this is a possibility following neck rejuvenation surgery. Many factors can contribute including your original anatomy, the extend of surgery performed by your surgeon, the technique used for your platysmaplasty and your healing. Depend g on the degree of banding, Botox may soften the pleating. While this is a non-operative option, it is a short term fix. Revision surgery may be needed. I would start with a follow up with your surgeon to see what options are available to you.best wishes!

Hi Lori, it is difficult to address your problem specifically without seeing pictures. It is possible to get recurrent banding following a platysmaplasty do to loosening of the middle portions of the muscle. If it is very minimal, Botox may be effective at correcting the problem. If it is more dramatic, you may need a revision tightening procedure. Follow up with your surgeon to discuss options.

Necking banding can be challenging to correct. However, the vertical lines in the upper neck should be improved after platysmaplasty or necklift. I am sorry that you are not happy with your results. Some patients will experience recurrence of their bands at some point of their lives.
