Would you recommend a 360 or an extended tummy tuck ? What about the thighs in ans out ? Which surgeries can be safely combined and what do you think will have the best outcome ? Thank you

In my experience, your best results would be a tummy tuck with Vaser ultrasound liposuction 360 including inner and outer thighs.

This will contour all of the stubborn areas, tighten skin of thighs, waist, back, torso.

Adding the tummy tuck will remove all lax belly skin pooch and around the belly button. This will also tighten lose muscles.

You would get improvement and some skin tightening with 360 alone, but limited results as compared with the tummy tuck.

We routinely perform these together with easier, combined recovery.

Thank you for your question and photos. First, liposuction 360 means removing fat from all around or circumferentially from the body. Based on these photos, without the advantage of a physical examination, you would get the best results with liposuction 360 degrees and a full abdominoplasty. Lipo 360 is not interchangeable with a tummy tuck procedure. They complement each other to deliver the best body shape based on your muscle and bony anatomy. I would recommend light liposuction of the medial thighs only. With the tummy tuck this fullness maybe greatly improved since the medial thigh tissues will be advanced superiorly. The incision will still stay low at the pubic bone level. See a board certified plastic surgeon for an in person consultation/evaluation. Good luck.

Lipo 360 and an extended tummy tuck treat different problems. Lipo 360 removes fat around the waist, flanks, abdomen, and back, but it does not remove loose skin or repair muscle separation. An extended tummy tuck removes loose abdominal and hip skin, tightens the abdominal wall if needed, and can smooth the front and sides more effectively.
From your question and photos, if there is significant loose lower abdominal skin, wrinkling, or overhang, an extended tummy tuck would likely give a more powerful result than liposuction alone. Lipo alone can sometimes make loose skin look worse if the skin does not retract well.
The thigh area needs a separate exam. If the issue is mainly fat and the skin quality is good, thigh liposuction may help. If there is true loose inner thigh skin, a thigh lift may be more effective, but it comes with longer scars and a more involved recovery.
Combining procedures depends on your health, BMI, operative time, blood clot risk, and how extensive each surgery is. A tummy tuck with some liposuction to the flanks is commonly combined, but adding major thigh surgery at the same time may be too much for some patients.
My advice would be to prioritize the abdomen and waist first. If loose skin is the main issue, consider an extended tummy tuck with selective liposuction rather than lipo 360 alone. Thigh liposuction or thigh lift can be planned separately or combined only if your surgeon feels the total surgery time and safety profile are appropriate.
