I was hit by a vehicle as a child and have suffered from facial asymmetry as I’ve gotten older. I notice one side of my nose has a bump and the other side is flat. I’m not sure if this is some type of scar tissue or cartilage issue. Can it be corrected with closed rhinoplasty if it isn’t scar tissue? I had septoplasty earlier this year for breathing issues but the bump has been here since college.

So, lets talk about closed vs. open rhinoplasty. There is indeed a lot of chatter about one being superior to another. The truth is, it depends on who is performing the surgery, and what needs to be done. If a surgeon is highly skilled and with decades of experience in rhinoplasty surgery, both are important approaches to correct different issues. I’ve performed countless closed rhinoplasties, and countless open rhinoplasties. Each has its benefits, and it is up to the surgeon to determine the appropriate approach. The techniques that I use – whether open or closed -- produce very little difference in healing time and recovery. So, the real question in your case is whether you would be a good candidate for rhinoplasty in general – no matter the approach. The answer, just based on your photos (though a thorough in-person examination would be needed) appears to be “yes.” The bump can be addressed whether it is the result of a fractured nasal bone or scar tissue.

Answer: Closed rhinoplasty Any type of rhinoplasty will be able to address your dorsal hump. It is too bad you did not mention it prior to your septoplasty. It is quite simple to address the two procedures but recovery from a simple hump reduction is quite easy.

Keep in mind that following advice from a surgeon on this or any other website who proposes to tell you whether you are an appropriate candidate for surgery and exactly what to do without examining you, physically feeling your tissue, assessing your desired outcome, taking a full medical history and discussing the pros and cons of the operative procedure would not be in your best interest. Natural appearing results need to be individualized and what is appropriate for one patient is not necessarily the best for someone else.
