Answer: Rhinoplasty A bent (deviated) columella is usually a sign of significant deviation of the caudal aspect of the nasal septum. Caudal aspect of the septum interplays with medial crus of the lower lateral cartilages of the nose on both sides to constitute the structural component of the nasal base. This complex relation warrants a thourogh nasal examination and knowledge of any pertinent previous history such as trauma or surgery before one could assess the prognosis and the extent of surgery needed to aim the desired improvement.

Answer: Rhinoplasty Consultation Thank you for sharing your question and photos. You are an excellent candidate for a rhinoplasty to refine your nose to better fit your other facial features. With regards to the asymmetry you are noticing at your nasal tip, this can certainly be significantly improved during a rhinoplasty. Depending on the degree or asymmetry, your surgeon will be able to evaluate what degree of improvement is attainable. On average, the cost of a rhinoplasty is around $8000-$10,000 and payment plans vary by office. If you have any functional issues, such as a deviated septum, a portion of your procedure may be covered by insurance. The next step would be to schedule a consultation with a board-certified facial plastic surgeon. I always recommend consulting with several surgeons before deciding who will perform your procedure. Best of luck!

Answer: Rhinoplasty for a more feminine and attractive nose. Rhinoplasty for a more feminine and attractive nose. You need increased projection of your tip, removal of your septal deviation, refinement of your tip, etc. Your right ala is much smaller than the left as well. This should be done by a VERY experienced rhinoplasty surgeon who can show you many photos of his work.

Answer: Rhinoplasty Hello and thank you for your question. Based on your photograph, you may benefit from a tip refining rhinoplasty. Your surgeon can accomplish this by trimming, suturing, and reshaping the cartilage in your lower nose. You may also benefit from a cephalic rotation of your tip to turn up your tip slightly and a conservative dorsal hump reduction. Your columellar asymmetry can also be addressed during this surgery also. Cost varies based on geographic region and surgeon expertise. Make sure you specifically look at before and after pictures of real patients who have had this surgery performed by your surgeon and not just a computer animation system. The most important aspect is to find a surgeon you are comfortable with. I recommend that you seek consultation with a qualified board-certified plastic surgeon who can evaluate you in person. Best wishes and good luck. Richard G. Reish, M.D. Harvard-trained plastic surgeon

Answer: Bent Columella Typically if the columella is deviated the underlying problem is deviation of the caudal ( bottom ) of the septum. The septum is the cartilage in the middle that divides the left and right side of the nasal passage. This acts as the beam that supports the outer cartilages that make up the shape of the nose. When the caudal septum is deviated, a reconstructive rhinoplasty is necessary to correct the problem. I have found that this is best accomplished in my hands by removing the deviated cartilage and then using the cartilage to reconstruct the septum back into the midline. This is call extracorporeal septoplasty with reconstruction using spreader grafts and a replacement caudal strut. Please seek out a surgeon that has extensive experience in rhinoplasty as this can be a challenging procedure. Please understand that there can be many reasons for the problem that you describe and a full exam is necessary to create an accurate treatment plan.
