You have a classic case of waterfall deformity. Your breast implants are higher on the chest wall and the breast tissue is falling down off of it. One alternative is explantation and lift using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery. Another alternative would be exchanging the implants for smaller ones and release the pocket inferiorly. The problem with inferior pocket release is the breast implants may bottom out and you may have a problem with a double bubble.

The lower side needs to be lifted with a lollipop type lift. Your best chance for symmetry would be to do the same to the other breast.

I would suggest doing the lift on the both breasts to raise the nipples and decreasing the implant size on the left. If you are happy with the size of the right, then keep that implant. I would keep both implants behind the muscle for better long-term support.

There is obviously some missing information in your very complicated breast augmentation history that is best obtained in person. The single photo is also inadequate to accurately assess your results. The one breast definitely needs a breast lift. An option would be to remove both implants and let things heal completely to see what you've got left to work with. Good luck!

Using excessively large implants causes increased, secondary side effects undesirable outcomes, and increases complication rates. It’s always better to stay with modest size implants, and that would be one good place to start. It looks like your left breast needs a lift.
A quality assessment requires quality information. I suggest relying on in person consultations to get a more accurate assessment and better understanding of treatment options.
For a second opinion, consultations, I recommend patients come prepared to bring with them complete sets of before, and after pictures for all previous procedures and operative reports for all previous operations.
