I got 300c mod+ subglandular implants 11 months ago. I've been sort of unsatisfied with how they look and i think its mostly because of the lack of underboob. it just looks like theyre there but haven't dropped (even though they did and fluffed as well). Is there any way to get the look from the wish pic? I've considered getting lower profile as well cus i like a flatter look but im not sure that addresses the lack of underboob.
Answer: Implants I would suggest placing the implants behind the muscle and perhaps going slightly smaller. Just placing the implants behind small breasts will give you the "balls on the chest" look, which you have now. You need more tissue to hide the implants. The other option is doing fat transfers to add more tissue to the upper breast. This may take more than one transfer.

Answer: Implants Implants in a subglandular position tend to have a more round "stuck on" look because there is minimal soft tissue helping smooth out the contour of the implants. I would recommend potentially changing to a submuscular dual plane pocket.

Answer: Subglandular Settling I would personally recommend giving your implants a few years to continue settling. Subglandular implants do not have the pectoralis muscle holding them up, so you will continue to drop as gravity pulls them. Most women, in fact, have this opposite issue, and want more upper pole fullness. I would not recommend doing a revision so soon, as the less trauma your new 'pocket' undergoes, the better longevity you will have with your implants.

Answer: Round subglandular breast implants Dear Luminous, Although there are some studies out that declare that even plastic surgeons can't tell a subglandular from a submuscular breast implant placement, sometimes you can. Your results would benefit from either a submuscular breast implant transposition, or an anatomic shaped breast implant. Either would give you a much more natural upper breast pole sloping and decreased rounded appearance. Good luck!

Answer: What determines breast augmentation outcomes? There are generally three variables that determine what breast augmentation outcomes will look like. The first one is the patient’s candidacy for the procedure. In your case, your breast fit slightly low on your chest wall. The breast have moderate distance from the midline, and there is slight divergence on one side(right side) the second variable is the choice of implant in regards to type shape and size. The third variable is placing the implant correctly in the correct anatomic location. Presley, I think it’s a pectoral implant placement is better and I think a sub pectoral placement would create a slightly improved breast contour. Your current implants are placed centrally under each breast, and I don’t think you should lower the implant pocket. Your outcome is in part, a reflection of your candidacy for the procedure, and I think to some degree the choice of placing the implant above the pectoralis muscle. I generally choose low profile implants unless the patient wants large implants and has a small breast diameter. Best, Mats Hagstrom, MD
