Thanks for your question and picture. Not only is a secondary mastopexy safe, I think you could expect an excellent result. Although removing all of the areola skin was not possible for the first surgery it will be easily done this time. Your surgeon would benefit from having a copy of the original operative note, although after 15 years almost any approach will be safe. This can be done safely however you should choose a board certified experienced plastic surgeon for this procedure. Depending upon your skin tone in many instances the previous pigmented scarred tissue can be removed. This can be discussed any more formal consultation and detailed physical examination.

Some very good questions Melan22,There is no risk of necrosis if the surgeon only removes the excess remaining areola and does not operate in the breast tissue. You have what is generally called "bottoming out" of the breasts.Best wishes in your journey. 

Revision breast lift surgery is not only very common and safe but in the hands of an experienced plastic surgeon, results can be excellent.  Its essential to seek the services of a plastic surgeon experienced in beast lift revision surgeon.  You will need to have an examination and comprehensive discussion of your surgical options and anticipated outcomes.

Consult with a board-certified plastic surgeon who get good breast lift results. The residual areola skin can likely be removed; definitive advice would necessitate in person evaluation. I do not see any reason why there would be a problem with a second breast lift.  It is important for the surgeon who examines you to determine the amount of remaining areolar tissue that can be removed.  It is always helpful if you could obtain the original operative report, but at 15 years that can be difficult.  Especially with secondary surgery seek out a highly experienced surgeon.  Best of luck

There should not be any problem with repeat mastopexy.  As a matter of fact mastopexies often need to be redone since it is common for breasts to develop recurrent sagging.

I see your problem, thanks for the pictures.  It is safe to have a second breast lift but blood and nerve supply to the nipple have to be protected.  Having a copy of your prior operative report would be helpful but that may be hard to obtain now that you are 15 out.  Choose a board certified plastic surgeon with lots of experience with breast surgery and should do fine.
