I've always been embarrassed by my abnormally large areolas and saggy breasts. Been considering an areola reduction and a breast lift but am scared of an even more botched result then what I naturally have.

It also appears that you have some sagging of your breast. You would likely benefit from a breast lift procedure.A breast lift procedure uses incisions to elevate the nipple and reshape the breast. The goal is to provide a more youthful appearing breast. A breast lift uses incisions around the areola (doughnut incision), and sometimes requires the addition of an incision from the areola to the crease below the breast (lollipop incision). In some situations a third incision within the breast crease is used (anchor incision). The incision around the areola allows your surgeon to decrease the size of the areola. You may have read about postoperative re-stretching of the areola. Most surgeons are aware of this possibility and use a permanent suture that is placed around the entire areola border. The goal of this suture is to prevent areola from restretching. If a permanent suture is used, this decreases the chance that your areolas would return to their original size.

Answer: Areola reduction You have a combination of ptotic/saggy breast and very large areala. A breast lift/mastopecy would address both issues and you should have both a nicer shaped and positioned breasts with smaller areola. You are a great candidate for the surgery.

Answer: A breast lift/reduction A breast lift/reduction is a common procedure with typically very happy patients. Volume/size is reduced and shape is improved. There may be an issue with some part of the pigmented areola showing. Your Board Certified plastic surgeon should be able to discuss that with you as well as the various styles of lift/reductions. Based on the size of the areola, you will need at least a vertical breast lift to accomplish this, and even so it may not be possible to get the areola as small as you might prefer, the there may be areolar pigment along the vertical incision. But a significant improvement can be achieved. No one can promise how much change will occur with pregnancy. All the best.
