Based on the photos, an augmentation alone would likely add volume, but it would not reliably lift the nipple position or tighten the lower breast skin. If the nipple and breast tissue sit low before surgery, placing an implant alone can sometimes create a larger but still droopy breast, or a shape where the implant sits higher than the natural breast tissue. A natural-looking result does not necessarily mean avoiding a lift. In many patients, the most natural result comes from using a modest implant for volume and a lift to center the nipple and reshape the lower breast. The exact scar pattern depends on your measurements, skin quality, nipple position, and how much reshaping is needed. If you strongly want to avoid lift scars, you should ask your surgeon to show you what augmentation alone can realistically accomplish and what compromises you would be accepting. In some borderline cases, a small implant can be reasonable, but if there is true ptosis, the tradeoff is usually lower nipple position and less predictable shape. An in-person exam with breast measurements is the best way to decide. Bring goal photos that show the natural look you want, and ask specifically whether augmentation alone would create the breast shape you want or simply make the current shape larger.
