What grade of ptosis do I have and what can be done to help my deflated breasts? (photos)

My main concern is deflation and lack of upper pole volume. As my breasts sit quite close I want to avoid low profile implants, but the high profiles to fit breasts with a wide base width seem to be almost double the volume recommended with an uplift. Would a moderate profile give me upper pole definition. If it needs to be a high profile for more definition will I end up with a massive gap if the width is too small. Should I go with a lift and risk implants that are too narrow, or just implant?
