I have had 2 breast surgeries. The first, a disaster with overdissected pockets and bottoming out. Years later I had a great revision with a wonderful (since retired surgeon). He did a great job and tightly sutured the bottomed out side after replacing the implants. I now have slight bottoming out on the other side, you can feel the implant base below the incison line and the nipple is rising. Can this be repaired without removing my implants? What is the easiest way?

Yes, you have bottomed out from the weight of such large and heavy implants. It would be best to revise your lower pocket, perhaps use mesh for more support, and go smaller so this does not happen again. The 525 cc implant size is too large and heavy for your tissue. Otherwise, wear a good...
I would recommend surgical revision with internal capsulorrhaphy +/- mesh and implant exchange. Using mesh will likely be successful to fix your issue, but only if you keep the same size or go smaller. With a larger implant it will put too much pressure on the repair and will likely bottom out...
The size you chose is probably too large and heavy on your tissue and is sagging. Ideally, you would go smaller and support your tissue with an internal bra of mesh. If you want to try and keep this size, you will need mesh.
