I'm transgender, I want a bigger buttock and hips, should I do implants under the buttock muscle first than fat transfer over the muscle? Also, Is the any way I can have my Lateral obliques muscle pull more medial to give me a smaller waist? With yummy tuck? Recommendations or suggestions please help...

Answer: Transgender BBL and Lipo 360 hello,Thank you for posting our photos and for asking these well thought our questions. I believe your best result would be achieved with Fat Transfer to buttocks, as opposed to buttock implants. I stopped performing buttock implants 15 years ago, as the complication rate did not justify the improvements. Even since this time, I perform only Fat transfer to the buttocks. It is your own tissue, natural, and this new adipose tissue graft provides increased vascularity in your buttocks. It is all good!You would have a great result with fat extraction of of abdomen, flanks and scapular (back) , with re-injection into the buttocks and hip area. Over the past 10 years, I have performed BBL on several transgender patients who have had fantastic results. This can be done in combination with other procedures too, at the same time.With regards to your abdomen, it would be best to have a physical examination to determine if any muscle tightening is warranted. If needed, this could be performed in the same operative session. I have done this combination procedure (BBL + TT ) for many many years. To reduce the risk of DVT and pulmonary embolism I use only IV Sedation instead of General Anesthesia and I use only Board Certified Anesthesiologists.When consulting surgeons, be sure to look for photos with your similar body type. (Photos) If the waist was smaller at one time, the waist can likely be reduced through liposuction of the abdomen, flanks, back, and love handles to those same measurements or smaller provided the muscular and fascial integrity has remained the same. The harvested fat can be transferred to the buttock and hips to make your waist look even smaller. I think that redistributing the fat from those areas to the hips and buttocks is a reasonable idea.

Answer: Transgender hyper feminization To get more extreme hyper feminization implants and fat transfer is probably the ideal outcome as long as everything goes according to plan. Gluteal and hip implants to have their sets of complications leading to implant failure and possible disfigurement. This needs to be taken into consideration when going for that option. It’s definitely possible to pull in the waist with muscle tightening. I know one plastic surgeon in orange county who does this to the point of bending ribs and patients losing weight because they simply can’t eat afterwards from the increased intra Abdominal pressure.I’m sure there are others who do this in an equally aggressive fashion. When I’ve seen this done the muscles are pulled to the point where the rectus muscles are basically buried so oblique to oblique In the mid line. This is done on women with muscle separation from previous pregnancies and probably wouldn’t be required for someone with undisturbed muscle Facia.In regards to gluteal volume I suggest interviewing numerous board-certified plastic surgeons who have extensive experience with implants as well as those who prefer fat transfer alone.Generally in the air I’m a big advocate of fat transfer and believe it to be a better choice for most than implants but when a substantial change is needed more than just desired implants may be something to look into.In regards to the tummy tuck many may question if the procedure is justified for muscle tightening only and someone will has normal undisturbed abdominal muscles.Even in the best circumstances it leaves a significant scar that Is significant though in your case you could have a scar that’s shorter than most tummy tuck incisions. These procedures are probably best done separately so you may want to look for plastic surgeons who specialize in each procedure.
