There are two ways the heart would go into arrest during surgery. The first one is due to coronary artery disease or blockage of the blood vessels supplying the heart. This is what happens during a heart attack. The heart doesn’t get enough oxygen and becomes ischemic. The second is in situations where the body is seriously under perfused because of a state of shock. During surgery this could happen from massive blood loss or under fluid resuscitation. This causes acid level that worsens with the release of intracellular potassium that eventually stop the heart. Cardiac arrest during surgery is not the problem but the end symptom. By the time someone arrests during surgery they are many other things that have gone wrong that represent the primary problem. These could have to do with things like having way too much surgery scheduled during a single procedure.Board certified plastic surgeons have a minimum of 10 years of medical training and sometimes more. This includes doing rotations in trauma surgery, critical care management, general surgery etc. etc. Many of us have done rotations in heart surgery.If you’re in the hands of a well trained physician who has experience and good clinical judgment the chance of serious complications area rare.The most serious complication that actually happens from a BBL is called a fat embolism. The consequence of this is often pulmonary failure leading to death. Eventually the heart stops but it is secondary to pulmonary failure. The incidence of fat embolism from a BBL has been described as being one in 3000.
