Since you are starting to show signs of aging I would recommend skin tightening and lifting procedures such as Ultherapy or Profound RF. Ultherapy is non-invasive and uses ultrasound technology to help build collagen. Profound uses microneedling with radiofrequency and is minimally invasive. There are excellent non-surgical skin tightening procedures that can firm up the neck and create a sleeker contour. These procedures can be combined with liposuction if necessary to remove isolated areas of fat but in your case, I see little fat. Based on an in-person exam, I would think a non-invasive treatment like Ulthera or Thermage would be best, coupled with a small amount of Botox in your platysmal muscle bands. Make sure you work with a board-certified dermatologist who is an expert in non-surgical facial contouring. At our office we recommend using Alastin Regenerating Skin Nectar two weeks before and after treatment for an improved result. You can also use Dysport, Botox, Xeomin or Jeuveau to help relax some of your platysmal bands as well!
Thank you for the question and photos. Ultherapy is a great skin tightening modality. There is some discomfort during the procedure which lasts for 1-2 hrs. Some patients have amazing results and it is non invasive. It is best to consult a board certified plastic surgeon. Good luck.
I may be the only physician on this site who has answered you in this way but as many cases of Ultherapy I have done since it first was FDA cleared, and many patients have received very good skin tightening, the photographs you have shown seem to indicate you do not have sagging inelastic skin, but the platysmal muscle bands have been forwardly displaced. I have not been successful tightening these bands with Ultherapy. Platysma bands are often improved through a surgical procedure, platysmaplasty. But this is a neck lift with downtime and some risks. Botox injections can in many patients make the muscle bands retract somewhat but never do they vanish. It has to be repeated every 4 months for most patients. Microneedling with radiofrequency can help some patients with hanging muscle bands. Thermitight is a non surgical, but minimally invasive procedure that does help some patients tighten their muscle bands but there is swelling and downtime often with bruising. You should consult, in person, with a board-certified dermatologist or plastic surgeon to examine your skin and determine what is the exact cause of your neck sagging.
