Hypertrophic/Keloids post Neck Lift. Are there any treatments I should be following? (Photo)

I have terrible post neck lift scarring. I have had three rounds of steroid injections, and this is the scarring at 4 months post op. I have never had scarring or keyloids before elsewhere. Besides injections and silicone, are there other treatments I should be following? How long before one can have scar revision surgery? Are there specialists for scarring? I welcome any and all advice. I feel like the scars are getting worse and larger, even since the beginning of steroid injections.

Thank you for the question. Steroids will help reduce raised areas and PDL therapy will reduce the redness. Keep going with the medical treatments and wait until at least one year post surgery, it will likely improve significantly. The decision to proceed with scar revision surgery is not to be taken lightly as the problem can reoccur afterwards. I hope this helps.

Hypertrophic scarring after a face/necklift results from tension placed on the incision.  For this reason revision surgery is often unsuccessful. Cutting out the scar and placing more tension on the incision will often result in worse scars.At this stage you need to control the hypertrophic process with steroid injections, I would recommend Kenalog 40mg/ml on a monthly basis until they begin to soften and regress.  Pulse dye lasers are also helpful in the treatment.

Unfortunately, scarring is a risk from necklift surgery. This can happen for a number of reasons, the most common being closure with too much tension on the skin. Pulsed-dye laser and 5-FU can provide some improvement. Ultimately, you may need scar revision surgery however you would want to wait at least a year before considering surgery. I hope this helps.

I agree with Dr. Glasgold - which is no surprise, considering he trained me years ago!Steroid injection, pulsed dye laser are good choices for now; I'd also add 5-FU injections to that list.Later, perhaps in a year or so, there will be some loosening in your neck to allow for further "neck lifting" to generate enough skin to close the incisions without tension. Revision sooner than that is unlikely to be as successful.
