I want implants with lots of upper fullness, but I don't want HP because I want them to be round. What are my options? (Photo)

Thank you for your question, but I have to disagree with almost every opinion expressed below. To get a round, fake appearance with maximal upper pole fullness, you need to go with an Ultra-High profile implant. The best on the market is the Allergan Inspira line as these are "dome" shaped and are overfilled without shell collapse. That is precisely why you don't want to go with an overfilled saline implant. Saline implants by their nature are less dome shaped and more saucer shaped. As you overfill them they become more saucer shaped lifting the edges off the chest wall and increasing their feel around the perimeter. As you go with larger round implants, the tissues are slightly thinned which increases the feel of the implant and with time, saline implant shells tend to stretch giving more rippling and wrinkling. Often times, I will use an implant significantly wider than the chest base width. As a breast revision specialist and having a significant number of my patients requesting a very round look, I have developed several strategies to achieve that look. Implants are three dimensional objects. The term 'round' describes a two dimensional shape. Among round devices, low profile are discoid in shape and high profile are spherical, with the intermediate profiles of many names somewhere in between.  The photos you post show relatively spherical breasts.  As for overfilling saline implants, consider that fact that when they are overfilled well beyond their recommended limits they become relatively firm and unyielding.  This will have a profound effect on your breast tissue over time, causing substantial thinning and loss of volume.  Remember this as it will potentially cause significant problems for you later.  Also remember that you are likely in safer hands with an ABPS certified/ASAPS member surgeon who is interested in your safety. Best of luck!

Higher profile implants generally look more "fake" because they are more round and more upper pole fullness.  A moderate plus saline implant is a nice implant as it has a wide range of fill volume and can achieve a larger size than a moderate or high profile implant.  A moderate implant tends to be flatter and a very wide implant.  Generally a moderate implant is not used for larger sizes.I would perhaps discuss your options with a board certified plastic surgeon who can examine you in person and discuss what your best option would be.Hope that helps.

If the photos are an indication of the look you want, then very large implants over the muscle is the way to go.
However, you don't provide any photos of your breasts and the degree of coverage of the implants has a lot to do with whether or not this can be achieved.
Also bear in mind that breasts like the ones in the photos have a very finite lifespan before they are droopy and require revision.

Thank you for your photo. Since your goal is to have very round and full breasts, a moderate plus profile implant could definitely work as long as you pick a relatively larger size that will fit with your body and physique. In my experience, the majority patients choose to go with moderate plus implants for both natural as well as that "round full" look. The exact implant size can vary depending on your taste, body measurements, current breast size and tissue quality, manufacturer, and style of implant. Therefore, a single "best" volume size cannot be determined by height and weight  alone. It is important to have an in-office examination where several measurements will be taken of both breasts and your chest anatomy to determine what range of implant sizes will "fit" on your body. Within that range, the style, projection, and shape can be further varied to customize your augmentation for your own aesthetic goals. Hope this helps!

Hello Everything depends on what is your expectation and if your profile would adapt large breasts can perform your surgery without any problem, in my case I only perform breast augmentation with implants, only with silicone implants, because these give more firmness to the Breasts You can opt instead for round-shaped silicone implants. There are several ways to get the fake look-either with overfilled saline implants, custom XL silicone implants or XL tissue custom expanders.It depends really on what cc size to want to get.
You can think of a saline implant as a balloon. The manufacturer recommends an overfill of about 20cc but they have been shown to be able to be filled much more than that. They do start loosing their shape and can significantly distort the appearance of your breasts. 
 I do overfill espcially since the FDA has restricted saline and silicone implants to a maximum saline shell size of 800cc and maximum silicone size of 800cc. I do not know why that number was picked arbitrarily. 
So, I do do breast reconstructions greater than that with XL tissue expanders which do pride a greater diameter and projection and upper pole fullness and allow the size to be adjusted either up or down. Also, the shell of the saline implants are very thin and can dreflate more easily. Since there are shortcomings in over filling saline implants, in my practice I would recommend mid-plus profile silicone implants that are on the large side for your breast tissue volume to give you greater upper pole fullness.  This can be tested during surgery with the use of sizers to determine the actual outcome of the procedure.  Keep in mind however that the larger and heavier the implant the greater the risk of future thinning of your breast tissue, stretching of your skin and loss of that upper pole fullness over time as your implant settles.  This often leads patients to return to their plastic surgeon seeking an even larger implant starting the process over again.  Remember that the photos you show are only taken at one point in time and those patients may or may not look as good two three or four years later.  Also overfilled saline implants become firm and develop scalloping on their edges.
