I am seeking permanent treatment to get rid of my eye bags and dark circles. For as long as I can remember, I've had bags and under-eye circles which is hereditary. I am 33 years old. Wearing makeup to cover them only helps a little and creams does nothing. I'm tired of looking like a raccoon with puffy bags! Can transconjunctival blepharoplasty help me achieve this result? I am not interested in fillers. Please advise.

Yes--a transconjunctival blepharoplasty will give you the result you want if you see a very experienced surgeon who can show you many patients who have had this surgery and look good after. Having done this for more than 35 years without any complication I can tell you this is normally a surgery without problems.

Answer: The answer very much depends on your actual examination. You do have fullness in an area that is commonly caused by herniated orbital fat. If this proves to be the case on examination, then yes, this can be very successfully managed with a transconjunctival lower blepharoplasty. Typically it is best to preserve the lower eyelid fat and use it to help add volume to the top to the cheek. That maneuver is called an arcus marginalia release. The reason why it is necessary to examine you is that the fullness can also be caused by a prominent orbital rim. In that case, fillers are a much better option.
