I am 35 and have "turkey neck" in my family. I also suffer from TMJ which I believe has caused me to develop a platysma band prematurely. I have had Botox which did help a bit. but because I also happen to store fat under my chin, it is making is x2 as bad. I have always looked younger than my age but when I look in the mirror now it is all I see!

since this is the non-surgical section question, the answer would be botulism toxin product (botox, dysport, xeomin etc).you will require repeated treatment but it would reduce the appeareance of the bands.

Thank you for your photograph and questions.  Botox is an excellent option initially to help treat platysmas bands, but if you have had poor results, I can understand your reluctance.  You may want to discuss with your injector a different injection pattern to perhaps avoid the the secondary effects to your chin.  The next best option to remove these permanent bands is a neck lift accessing the middle part of your neck through an incision underneath your chin to correct the underlying muscle that is causing you to have this banding.  As you have limited banding this could be done quickly and with little downtime.  See a board certified plastic surgeon for an in-person examination and the best treatment options.

Without examining you it's difficult to be entirely certain but Botox is certainly a good place to start for visible bands of the platysma. If you have fat collecting under your chin then you should consider a treatment with something like MiniFX which can tighten the skin and kill excess fat nonsurgically. Microliposuction of the neck may also be an option. The only way to know for sure is to book a consultation with a board-certified plastic surgeon near you to see what your options are. I would suggest Ultherapy the non invasive ultrasound treatment to tighten, tone and reduce the fat volume in your neck.  This may then either eliminate the need for Botox for the Platysma band or reduce the amount of units needed.  Ultherapy treatments last up to 3 years versus 3 months for Botox.

I would consider the use of Botox or Dysport.  I would also consider liposuction under the neck through a small incision if you are concerned about the fatty tissue.  Alternatively you can consider Ulthera under the neck to tighten the neck tissue

There are not a lot of non-surgical options for treating the platysma bands. Botox is your best bet. In order to maintain the results, you will need to be treated every 4-6 months.

Surgery via tightening of the muscle bands (platysmaplasty) and removal of fat under the chin either through liposuction or direct excision of the fat under the muscles (excision of subplatysmal fat) will give you the long term result you are looking for and will require some downtime for the procedure. A non-surgical alternative at your age is also just as effective but will require retreatment in a few years. The alternative would be to use Botox for the platysma bands and radiofrequency technology (e.g. Vector RF, Thermi RF, Exilis Elite, Thermage, etc.) to melt fat and tighten the skin. Ulthera will not melt fat. There is also an injection for submental (under chin) fat that may be helpful that has recently been FDA approved called Kybella. Whichever treatment you decide, know that the most successful would be a surgical option with its tradeoff being an incision and downtime.
