A procedure my patients love involves a combination of juvederm filler along the lines and corner of your mouth coupled with a small amount of botox to relax the depressor angularis oris (DAO's) to reduce the appreance of the permafrown. I recommend a combination approach to get the most improvement for this area. I recommend fillers for the folds themselves but also lifting around the mouth and cheeks to build underlying anatomical support. In our practice we combine sculptra with other fillers "layered" on top and lasers, rf treatments, J Plasma and peels to get the most conservative natural look. Cost will vary. There are various treatment options depending on multiple factors such as how deep the wrinkles are, whether the lines are there at rest or only with movement, etc. I'd suggest a proper in-person assessment with your dermatologist. A filler is often a great option (e.g. Restylane fine lines, Juvederm Ultra or Volbella, Redensity-II), sometimes alone or with a little bit of Botox. Laser resurfacing or microneedling are also good options for improving the texture/tone of the skin around the mouth. Cost is variable, depending on how much product is needed, which treatment is selected, etc. The choice of an expert physician injector such as a dermatologist to optimize safety and to give the best, natural looking and symmetric results possible is the most important factor.
Flexible gel hyaluronic acid based fillers like Restylane Refyne/Defyne work well for laugh lines (nasolabial folds, marionette lines) while softer HA filler like Restylane Silk work well for perioral lip lines and thicker HA fillers like Restylane Lyft work for the jawline and chin.
It is difficult to be certain of your particular problem without the photos. I assume you are referring to the "smokers" lines. These are frequently associated with the loss of volume in the entire area. There are many options offered everywhere but you must remember that if your lines are very significant the chance of re-creating a smooth wrinkle-less lips is truly minimal regardless of the technique. One option, and the least expensive, is a trial of Botox. Due to dose limitation in this area, the optimal results can be seen after 3-4 injections. Fillers can correct the lines quite successfully but it is only a temporary solution. Laser or radio-frequency treatments may also improve the appearance of your lines. The combination of Botox and either laser or aggressive RF micro needling could be very satisfying. Again, start slowly with Botox. If placed properly you may be very pleased. Also, remember that these lines are caused by repetitive contractions of the circular muscle around your mouth. To minimize these movements (e.g.. limiting use of a straw while drinking liquids) will be of help. Good luck!
