I got my Natrelle gel implants in 2016 when I was 18 years old. I’ve had 2 babies in the last 3 years and this is what my breasts look like now. I’m curious if I need a breast lift with new implants or if just one of those options would suffice to fix the appearance. I’m 26 years old now and embarrassed for my husband to see me topless.

Answer: Revision surgery Dear Creative63713, determining whether you need simply a breast augmentation versus a breast lift can be somewhat complicated. It depends on a number of factors including skin laxity and current nipple position. Generally speaking, if the nipples are lower than the inframammary line the patient will most likely need a breast lift. If the nipples are at or above the inframammary line we can generally get by with just a breast augmentation depending on the size of the implant the patient wants. I often still using implant when I perform a breast lift because it provides more upper breast fullness after the lift. To be sure a consultation with a board-certified plastic surgeon will help determine what the patient actually needs. Answer: Breast lift I think that you could achieve a more youthful look if you have a breast lift to elevate your nipple and to remove some of the sagging skin. You have the option to change your implants if you want them to be larger or smaller, or even if you just want to update them since they are 7-8 years old. Talk to your surgeon and then you will have a better idea of what to expect with your body. Best of luck!

Answer: Revision or explant with Bellesoma Method Your breasts are at 2 different levels, your implants are high and your breast tissue is low. They need to be repositioned and realigned. This can be done through a circumareola approach to reposition the nipple-areola complex higher and lower the implants. The other alternative would be explantation with lift using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery. Later, fat transfers can be performed if additional volume is desired. Best Wishes, Gary Horndeski, M.D.
