A little over 6 months ago I had a BA. I got 485cc implants subfascial OTM. My concern is that I have lost all of the upper pole. Any advice on how this could be fixed? I have moved and have no interest in traveling back to see my surgeon. I’m curious to what others think before I find a local surgeon near me.
Answer: "How this could be fixed?" I would recommend downsizing implants to a lower profile, placing internal mesh (Galaflex) for support, and performing a lift. The large implants have further stretched out your breasts. This is the best plan in my opinion to provide a reliable, long-term result.

Answer: Implants Your implants have dropped down some. they could be placed behind the muscle for better support and use mesh if needed. Right now, wear good supporting bras.

Answer: Breast revision Thank you for your question. I see this problem all of the time. A subfascial breast augmentation is equivalent to a subglandular breast augmentation. There is literally no support for your breast. I have a whole gallery of chronic changes associated with implants on top of the muscle. Gravity always wins. A large portion of my practice is dedicated to revision breast surgery, and I have been in practice for over 28 years. I wrote the only paper written on going from on top to below the muscle which would add support to your breasts. In order to keep the upper pole fullness and have an attractive breast, you would need to go under the muscle and redo the lift. Best wishes to you.

Answer: Revision In your before photograph, your breasts were low on the chest wall. You would have benefited from a circumareola approach to move the nipple-areola complex higher. At this time, your implants are large, low and you will need a breast lift. Most likely you will need plication of the capsules inferiorly and laterally to move the breasts more higher and medially.
