I’m a female 5’4” 153lb and had a breast augmentation in 2006 but I’m looking to find the right surgeon in Portland,OROne of my questions is, my previous surgeon had put the Saline implants behind the muscle, which caused the breasts to severely fall to the side when laying down. When I get my next surgery augmentation from 36C to D and a lift I’m guessing… can my new surgeon put the new implants over the muscle? Is there going to be saggy skin on the side or will the new implant fill that in?

Your implants have dropped down some. they could be placed behind the muscle for better support and use mesh if needed. Right now, wear good supporting bras.
If you are small, your tissue may not support heavy implants. Measurements and an examination is needed to see what could be your maximum size and still look good long term. Usually, 250-300 cc's is the maximum for most women like you.
You did not post your before photos, which would be very helpful. Some women whose breasts are wide apart will always have wide apart breasts. The bases cannot be moved. Sometimes the out pockets can be tightened to help decrease the lateral movement and fat can be added to the medial pockets...
