50yo post breastfeeding, petite, 105 lb, 5'3. Had surgery 4 mos ago for h/o asymmetry & gr 2 ptosis -300 cc implants under each muscle. I now have waterfall effect. Larger breast still has gr 2 ptosis, other w/grade 1. Breast tissue sits at bottom of implant. When I bend forward in front of a mirror, I can see my breast tissue sagging in front of the implant. During a mastopexy can the surgeon remove excess tissue in the breasts, especially larger breast (which just has more sagging skin/tissue) to even out size/symmetry/sagging?

Answer: Breast lift with tissue removal needed. I would start by being sure that the bottom of your implant sits on your inframammary fold. If it does, and the implants are symmetrical with regard to position, then a breast lift to reposition your nipple and areola to the midpoint of the implant should be done to give you the best aesthetic result. If there is a long distance between your nipple to your fold, this is the tissue which hangs off the implant. Tucking that tissue up inside the breast lift pattern can lead to early recurrence of the waterfall effect, so to remedy that, resection of the tissue directly underneath the areola removes the tissue which would hang and tightens the breast tissue, which is in addition to the skin tightening of a lift. Have a detailed consultation with your plastic surgeon regarding this. Best wishes.

Answer: Waterfall deformity It happens waterfall deformity after breast augmentatıon . The areola must be positioned up and the breast tissue hanging down must be removed with breast lift operation. Best Wishes Erman Ak MD
