I had a breast lift/reduction that I am not happy with and it is affecting my confidence, my self-esteem and most importantly my career. My concerns are areolas, as the surgeon did not reduce them down while reducing my breasts. My scars are too low on my ribs and I cannot hide them, and something went off with my nipples. How can this be carefully corrected? Do I now have inverted nipples? What can I do to fix this? I desperately need this fixed. Please help
Answer: A scar revision for the nipples can be done. Treat scars with lasers and melarase creams to help reduce appearance. Plastic surgery scars require attention after surgery so that they remain cosmetically-appealing and barely noticeable. In our office, we use a protocol for scars that minimizes their activity in the 6-8 weeks after surgery. If you have late plastic surgery scars, you may also require laser and RF therapy to help reduce the longterm effects of the scarring. Keloids, hypertrophic scars, widened scars, red scars, and hyperpigmented scars all require scar modulation with topical creams followed by lasers and RF therapy. Topical skincare should be directed to a specific need, whether for pigmentation or hypertrophy and our office can guide you through the process to obtain the correct Plato’s Scar cream or Melarase cream for hypertrophy and discoloration. In terms of lasers, we would employ the vbeam laser for redness, Venus Viva RF, PRP microneedling, chemical peels, and Morpheus 8, fractional erbium, or fractional co2. All of these have their individual benefits. Most of all, we recommend you visit a solid plastic surgeon who understands surgical scar revision, laser therapy, and RF therapy for scars. Answer: Breast lift & reduction Dear Intuitive2337, I understand your concern. However, without a proper assessment, it would be difficult to determine what is wrong. It is best that you visit your plastic surgeon for further assessment or ask for a second opinion. Only after a thorough examination, you can get proper recommendations and advice. Answer: Requirement for additional surgery: There are many factors that affect the long term results of breast reduction surgery. Risks of breast reduction surgeries: As with any surgery, breast reduction surgery has some risks. Although most people with this surgery do not have problems related to surgery, it is important for you to know them. In your personal choice, it is important that you know the risks of surgery and the benefits it will provide you. When your doctor tells you about these risks, you should discuss them all one by one. Bleeding: It is a rare condition, it can occur during surgery or in the postoperative period. After the surgery, the drain placed on the breasts is both to follow whether there is bleeding and to prevent a little blood from collecting in the breast tissue. Do not take aspirin or similar drugs from 10 days before the surgery, these drugs may increase the tendency to bleed. Infection: It is an extremely unusual condition. Most surgeons use protective antibiotics right at the beginning of surgery. Change in nipple and skin sensitivity: A temporary decrease in the sensitivity of these areas may occur. Scars: Each surgical procedure heals by leaving scars, most of the time the nature of the scars cannot be determined in advance. While scars are not noticed in some cases, in some patients may occur in the skin and deep tissues, and in some patients it may even be necessary to resort to surgical correction or other treatments. Dissatisfied with the result: You may not be satisfied with the size and shape of your breast. There is such a possibility, albeit very few. Pain: After breast reduction surgery, your neck, back and shoulder pain may continue. In rare cases, the nedbes that occur in the deep tissues of the skin and breast can cause pain. Hardness in the breast: Hardness due to nedbe or fat necrosis in the breast can be rare. This is an unpredictable situation. Delay in wound healing: It can be seen, albeit rarely. Some areas of the breast skin or nipple may take more time to fully recover. Frequent dressings may be required. Wound healing problems are more common in those with smoking habits. Asymmetry: Most women naturally have breast asymmetry. There may be differences in the shape, size and symmetry of the breast and nipple. Breast diseases: Breast disease or breast cancer may occur at any time of a person's life, regardless of breast reduction surgery. The person should have regular breast checks with or without surgery. Breastfeeding: Some women who have had breast reduction surgery may breastfeed their children, but usually this can be difficult to determine. If you want to have children and breastfeed, you should discuss this situation with your surgeon before having breast reduction surgery. Allergic reactions: It is known that local allergy to flastere, suture materials or pomades used, albeit rarely develop. Additional surgical necessity: There are many factors that affect the long-term results of breast reduction surgery. I totally understand your concerns areolas, and you need a minor surgical procedure again for this If a condition such as laxity and sagging occurs in the breasts, additional surgery may be required.

Answer: Plastic surgery out Plastic surgery outcomes are based on patient, candidacy and skill of the provider. I don’t understand why you had a breast reduction when you have implants. I’m guessing you went down in implant size. There really wasn’t an indication of doing a breast lift. In a nutshell, your provider selection was not well chosen, and at this point, it’s going to be difficult to improve your outcome. Your current candidacy for a quality outcome is much less than it was earlier. Choosing the right plastic surgeon is very important. To improve your outcome, you’ll need revision surgery, but the scars on your chest are not going to go away. Be more careful when selecting providers. I generally always recommend people have multiple in person consultations before selecting a surgeon. During each in person consultation, ask each provider to open up their portfolio and show you their entire collection of before and after pictures of previous patient had the same condition or physical characteristics as your own. An experienced plastic surgeon should have no difficulty showing you before and after pictures of at least 50 previous patients. Being shown a handful of pre-selected images, representing only the best results of their career may be insufficient. I generally recommend patients have multiple consultations before selecting a provider. I also recommend patients avoid virtual consultations whenever possible. Best, Mats Hagström MD
