I’ve had a few consults and get such a wide variation on recommended procedures. I’ve lost over 90lbs naturally with clean diet and CrossFit 5x+ a week. I’m currently 5’8 and 240lbs which puts my bmi around 36. The doctors I’ve consulted with state I don’t look like I weigh what I do and state losing more weight will impact results. Shouldn’t I lose more weight before surgery? Also what surgeries are recommended? Extended TT or body lift? I’d like MMO with no implants. Opinions please! Thank you
Answer: I would want to hear your top 3 concerns I would recommend achieving a steady state for 3-6 months prior to proceeding with surgery. Based on your pictures I do not know what your concerns are. With massive weight loss the common concerns become excess skin and deflation of breast tissue. You would need to reposition your breasts in the form of a lift to address the skin. The deflation can be best addressed with an implant, however that depends on your aesthetic goal and needs! Regarding the torso you have excess skin in the vertical and horizontal vectors. This may even be covered under insurance in your case. If you are scar averse a horizontal incision and traditional tummy-tuck/abdominoplasty. If you want to be as tight as possible and do not mind a midline scar then a fleur-de-lis abdominoplasty. Regarding the posterior torso, if you feel you have decent of buttock tissue you do a lift. However, I would be curious if LIPO 360/BBL as a second stage would be more in line with your goals? This really depends on your needs and aesthetic goals. In conjunction with a physical exam a board certified plastic surgeon can help guide you in this process. Hope that helps. Answer: Body contouring It appears you have had massive weight loss - congratulations! Great job doing it with diet and exercise.If you are at your nadir of weight loss (low point) and weight has been stable for 6 months and no other medical problems (heart disease, high BP, diabetes, vitamin deficiency, etc) then you could proceed with body contouring procedures. You will likely benefit from a breast lift +/- implants or potentially fat grafting in a 2nd surgery to augment the upper pole of the breasts.You will likely benefit from a circumferential body lift (360 degrees liposuction, belt lipectomy and incision all the way around waistline and back to tighten and lift the buttock, lateral thighs, flanks, back, bra lines and abdomen/pubic areas). You will likely benefit from a staged thigh lift as well (liposuction to debulk and sculpt the inner, outer and anterior and posterior thights) and then come back in 6 months and perform a formal thigh lift. These are long extensive surgeries but they will best deal with the loose skin and irregular contours. Many surgeons and facilities have BMI cutoffs of 30-35, and while may seem arbitrary, there is some data showing increased risk of complications for patients with higher BMI's. If you are able to to lose more weight, and are not yet at your low point/nadir of weight loss journey, then it is best to wait so you don't deflate further after surgery.
