You should do a liposuction 360 and have just a smaller/lean BBL. Meaning narrowing your waist and back will make your butt appear bigger and add some fat just to give you a more youthful appearance, Your breast can be done as well, remove the implants and fat graft your breast. You can lay on your side after surgery. One surgery is doable. Get a mammogram prior to Surgery for baseline.

You mentioned that the breasts have been giving you the most issues. It sounds like you may have a capsular contracture. I can not appreciate it in the pictures alone, I would need to examine you. Basically you have to form scar tissue around the implant, that is what defines a breast capsule. That scar tissue if robust can compress the implant, distort it and cause pain. Depending on when you had the implants placed, you may be eligible for a revision and benefits under warranty.
Regarding the buttock augmentation. I think that a board certified plastic surgeon can guide you in the process and see if you can create more contour and shape with liposuction and a smaller fat transfer. The back liposuction combined with the BBL can help curve and improve your shape. You can choose a more natural look or a more hourglass one.

Liposuction of torso is definitelly procedure which will create major improvement. Whether you would like some shaping of relatively flat butts in the same time, with fat transfer, is an issue that you have to decide. This is best done in communication with a surgeon. Issue of the breast needs to be evaluated and appropriate plan of treatment selected. It is possible to do all of these procedures in the same time since after liposuction and fat transfer your do not need to lay on your stomack. You may have a capsule with the one breast which may need surgery. At the same time, you can add liposuction if you want.

I encourage you to address your breasts first since they are causing issues. You can also combine liposuction with that. You certainly can have liposuction without a BBL. In fact there are techniques that I use to sculpt your curves with liposuction to improve the backside appearance without a BBL. See a board certified plastic surgeon near you to learn more.

Hi Gregarious255302, thank you for the great questions. If one of your breasts are painful, I would address that first. Some pain can be addressed by surgery, and some cannot. For example, pain caused by capsular contracture (or scar tissue around the implant) can be addressed with surgery. You can certainly have liposuction without fat grafting to the buttocks. However, if you're interested in a BBL, I would not get liposuction at the same time as the the breast surgery because you may want to "save" the fat down the line for your BBL.

You are a very good candidate for the BBL procedure. You may have your breast revision performed at the same time.
