I'm thinking of a syringe sucking out some of the fat. I am a little worried about taking out the entire sack due to hollowness. Can this be done?

Answer: Eyelid Fat There are two options to deal with fat under the eyes. From a non-surgical standpoint, you can try to add volume (filler) in the tear trough to camouflage the fat bag. The filler is not permanent but can be done the same day in the office. The more permanent solution is to perform a lower eyelid blepharoplasty with fat transposition. This will remove the bag but also avoid hollowness following surgery. Best to see an eyelid expert for an in person consultation.

There also s no guarantee that you will not have irregular lumpy areas under the eyelid skin, which happens to be the thinnest skin that we have on our body, by the way. Irregularities show through the skin very easily. My strong preference is to perform a lower eyelid fat pocket thinning with a reset of location, either by itself or with a mid cheek lift performed as well.
