Hi, For the past two years, I have had fat in the area above my upper eyelids, that area of skin that is usually recessed under the eyebrow in most people and was that same way with me. It started in my right eye only and would appear and then disappear. Then gradually, it remained and now affects both of my eyes. The fat, if that is what it is, is located closer to the inside of my eye as opposed to the outside of my eye. What would cause this?

Answer: Fat inside the upper eyelids The issue of puffy upper eyelids is best addressed with blepharoplasty. The fat is reduced leading to a more awake appearance. It is important to remove enough fat but not too much in order to prevent the undesireable hollowed-out appearance.

Answer: Fat in Upper Eyelids Thank you for your question Dave. What you describe is very common and happens as we age with varying degrees and varying rates in different people. Typically the upper eyelid skin tends to become loose at the rate of about 1mm per year starting in our early forties on average. On top of that the eyelid muscle becomes a little weaker as well and allows our orbital fat pads to prolapse forwards. You are describing the medial fat pad prolapsing.It would be very important for you to have a thorough examination by an experienced, Board Certified Facial Plastic Surgeon to determine if a Blepharoplasty is the right treatment for you (very likely) or if you can get away with non-invasive modalities. In order to make that decision, a thorough exam is necessary of your eyelids and brow and the surgeon you choose, needs to be very experienced both in invasive and non-invasive modalities. Our patients heal very quickly from blepharoplasty and very seldomly show any bruising. The stitches we perform are internal. The upper lid has two areas that the fat surrounding the eye can protrude, call the medial and the middle fat pads. This is genetic but also becomes more noticeable with age. This condition is easily corrected with a blepharoplasty in which some excess skin can be removed in addition to the fat pads. Consult with a experienced facial plastic surgeon.

The area you are questioning has a fat pocket and probably for genetic and aging reasons has prolapsed forward. It can be removed during a blepharoplasty. Skin and underlying fat is excised during this procedure. The recovery time is about 2 weeks and results are usually excellent. You should select an experienced Oculoplastic Surgeon to meet for consultation.
