Do I need a full blown lift or can I do donut? (Photos)

Based on your posted photos- it appears that you would benefit from Mastopexy with possible implants. Anything short of this in my opinion will not provide the best result. I suggest that you seek an experienced Board Certified PS

A donut type lift will provide some correction to the sagging but the appearance will not be ideal. A standard lift will provide complete correction, but will add extra scarring. Seek a consultation with a board certified plastic surgeon who can show you the differences you can achieve with these procedures as well as the difference in scarring.

In my opinion, a mastopexy (breast lift) combined with augmentation (either with fat or with implants) would best help get to towards your goals. The position of your nipple is below your breast folds and you need a vertical incision to help move that to a more youthful position. Depending on how much fuller and larger you wished to be, fat grafting or an implant would help restore volume to your upper poles and provide a rounder breast with more cleavage.

Thank you for your questions & photos. This is a common complaint from mothers after having children. To get more perky breast with the internal support, the anchor scar would most likely be necessary. The "donut lift" will give minimal lift with tension on the areola that can lead to stretching out again over time. For upper pole fullness you can transfer your own fat to your breast or a breast implant can be placed to achieve fuller rounder breasts. Your expectations and health will dictate the best procedure(s) for you. I suggest a consultation with a board certified plastic surgeon to go over the options that best address your goals. Implant-based breast enhancement is the best way to achieve upper pole fullness. Combining a sub glandular breast implant with a donut-type or peri areolar lift can achieve an improved nipple position and re-inflate the breast in women who only need a modest improvement in nipple position. This is an option that can be used to avoid the vertical lift scar.

Based on your photos, you appear to be a good candidate for a mastopexy (breast lift) which will put your nipples into the correct position, decrease the size of nipple/areola and allow excess skin to be taken away, giving you a more youthful looking breast. Some people are a little apprehensive to the full breast lift due to the scars, but typically they do very well.
Donut breast lift can help decrease the size of the areola and elevate the nipples, but it will not change the overall shape of the breasts.
Sometimes, breast implants can also be added in order to increase upper pole breast volume.
I would encourage you to seek a consultation with a Board-Certified Plastic Surgeon to further discuss your options, specific techniques, concerns and expectations.

The traditional donut lift will not be sufficient for your 34 DD bra. The technique I recommend is The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained.

my preferred lift is a lollipop lift when a periareolar won't do (ie greater than 2cm lift required). The lollipop technique was created by a Canadian plastic surgeon named Elizabeth Hall-Findley and treats the breast as a three dimensional structure instead of two dimensions (anchor - lift). Most importantly it does not require the horizontal component scars that the anchor-lift requires and breasts appear much more natural, lifted, and less boxy. Lastly, the lift lasts longer because the lollipop doesn't rely on the skin to hold the breast up, its actually breast tissue thats being brought together to support the breast and hence improves longevity. If you are considering surgery, I would suggest you consult a board-certified plastic surgeon. Only after a thorough examination, you will get more information and recommendations.
