Dear doctors, I had a dual plane breast augmentation 10 months ago and I was wondering how safe it is to get a fat transfer when the implants are already in. While I am very pleased with the surgery‘s positive outcome (no capsular contracture, no rippling, minimal animation deformity), I’m afraid I did not choose the right implants for the look that I wanted, and I’m feeling bothered by the the roundness at the top. I would love to use the fat to conceal the lines that are quite harsh in natural light, as well as filling up my chest area that always been very boney and skinny, with the sternum showing. How big is the risk of trigge capsular contracture if the body doesn’t accept the fat well? How does the needle or cannula not disturb the capsule around the implabts? Thank you for your answers.

Thank you for the question. Fat transfer over the implant can be performed safely and is indicated for the right patient candidate. However, make sure that this is the surgery to meet your goals by having a detailed discussion with your surgeon.

I agree that you have a very nice aesthetic result. If you are considering fat transfer to the breasts, I would recommend finding a surgeon who performs ultrasound-guided fat transfer. It would be easier and safer to perform fat transfer to the breasts if your implants are positioned underneath the muscle. Using an ultrasound, the surgeon can ensure that the cannula is positioned in the appropriate layer, can avoid injecting fat into the muscle, and can avoid damaging the implant.
If your implants are on top of the muscle, it isn't impossible to do fat transfer to the breasts, but is has more risks. In this instance, there is less tissue covering the implant, so there is less margin for error. The fat would need to be injected very precisely. And, as you mentioned, injecting fat can lead to things like inflammation, oil cysts, and/or infection, which obviously would not be ideal right next tot he implant or capsule. 
Based on the pictures you submitted, you appear very athletic, so you and your surgeon will need to find an area of your body from which to obtain an adequate volume of fat to do fat transfer. Alloclae is steriled donor fat that can be used for the same purpose and can be injected under local anesthesia and obviously negate the need for liposuction, but it is very very expensive.
