Breast fat transfer - Is it possible to achieve 130 cm (P-cup range) sized breasts without implants?

Hi! This might be a silly curiosity question, but I've always wondered: With multiple fat-transfer breast augmentations (fat grafting only, no implants), is it medically possible to reach very large, round breasts - around a 130 cm bust (P-cup range)? Or are there biological limits that prevent that?

The limiting factors for fat transfer to the breasts (or anywhere else) are the areas from which fat can be harvested and the amount of fat that will actually survive the transfer process.
In most patients, there is only so much fat that can be removed with liposuction before that area is "overdone." There's a balance between taking enough fat to transfer and achieving an aesthetic result vs. taking as much fat as you possibly can, which can result in contour deformities and/or loose skin in that area.
When we transfer fat to another area, that fat has to become vascularized. If there isn't a blood supply to the transferred fat, then it doesn't survive. So even under the best possible circumstances, 100% of the fat that is transferred is not going to survive. Most of the literatures says that somewhere between 50-70% of the fat that is transferred will 'take' and survive the fat transfer process.
So, to answer your question, a patient would need an adequate volume of fat and also start out with a decent breast size to achieve "very large, round breasts." Realistically, most patients will achieve about a 1 to 1.5 cup size increase with fat transfer to the breasts. In addition to that, fat transfer doesn't create the same appearance to the breasts as implants do. Fat transfer is good for adding volume, but not the best about creating shape (more specifically, upper pole fullness). Implants, on the other hand, are good for adding volume and providing a reliable shape to the breast, because the silicone/saline is contained with the defined shape and size of the implant shell.
