I lost 50 pounds. I have 45 more to go but already have a lot of loose skin hanging off my stomach and breasts so am looking into a tummy tuck and breast lift. My butt has rolls of loose skin now and is extremely flat. I have a lot of loose skin on my hips too. Will the tummy tuck procedure help the loose hip and butt cheek skin, or will I just have a flat stomach with rolls of skin hanging off my butt? My upper thighs and pubic area are all loose skin as well. Are these addressed?

Answer: Tummy tuck Congratulations on your impressive weight loss! A tummy tuck will address the upper and lower abdomen. An extended tummy tuck may also incorporate loose skin on the hips. If the mons area is full this may be incorporated into a tummy tuck. If your concern includes the buttocks area as well you may want to consider a circumferential tummy tuck/lower body lift. Depending on the laxity of the legs you may consider a thigh lift. It is common after losing significant weight to break the procedures up into stages. I've included a video on plastic surgery after major weight loss that I hope you find helpful.

Answer: Comprehensive Skin Tightening with a 360 Lower Body Lift Dear Cara1180, Firstly, congratulations on your weight loss journey, it's a commendable achievement and shows your dedication to improving your health and wellbeing. From your description, it sounds like you might benefit more from a 360 lower body lift rather than just a tummy tuck. A 360 lower body lift, also known as circumferential body lift or belt lipectomy, addresses loose skin not just in the abdominal area, but also around the hips, back, buttocks, and even the outer thighs. In contrast to a traditional tummy tuck, which primarily tightens the skin in the front of the abdomen, a 360 lower body lift extends the incision around your entire midsection. This allows for removal of the excess skin and tightening of the remaining skin all around, giving a more comprehensive and balanced improvement. It can remarkably improve the contours of your lower body, addressing the loose skin in your buttock, crotch, and thigh areas. I have performed many of these procedures, particularly on patients who have undergone significant weight loss and are dealing with extensive skin laxity. I would encourage you to look at our patient gallery to get an idea of what kind of results are achievable with this procedure. A separate procedure or a staged procedure would be necessary to address the breast lift you mentioned, as combining upper and lower body procedures can complicate the recovery process. Please consult with a board-certified plastic surgeon who specializes in body contouring after weight loss to discuss your options and formulate a plan that best meets your goals. I hope this helps, and I wish you the best in your body transformation journey. Sincerely, Dr. Katzen
