To get technical, an extended deep plane lower face and neck lift with deep neck sculpting/platysmaplasty would address a majority of your concerns. Your surgeon may also suggest a lower lid blepharoplasty to improve the undereye area. These procedures are unlikely to help with an uneven smile.
