Best of the Best! Incredible experience all round and natural looking results. I went to see Dr Julian De Silva for a deep plane facelift, upper and lower bleph and eyebrow lift for Ptosis. I had originally seen Dr Julian De Silva for a consultation back in 2017 and had followed his work for years, so I was familiar with his style of surgery and artistry. I had studied his work of the ‘Golden Ratio’ and even bought his book years ago. When I turned 40, I started noticing my jowls getting really bad (I had actually noticed these jowls slowly forming at the age of 33) , loose skin over my eyelids to the extent that I could no longer wear eyeshadow any longer, as it just creased everytime I tried to wear it. It really started to affect my confidence and I didn’t like looking in the mirror, I was just looking tired. Over the last 5 years, I have had botox and filler, but it got to a stage where it was not lifting my face in the way it used to when I was younger. At my first consultation, Dr Julian and his team immediately made me feel at ease. His wonderful team - Sarah, Mags, Mackez, Adam and the lovely nurses Alice, Josie and Kelly were all very kind and welcoming. I instantly felt at home. It’s a lovely energy when you go into 23 Harley Street and everything, including the operating theater is there. Dr Julian De Silva’s kind, humble and calm nature made him stand out from the rest of surgeons that I had seen over the years. I didn't feel rushed, and he listened and understood my concerns. He explained to me about hyper mobility, where the skin can stretch at a younger age and this made sense why I was seeing premature signs of aging such as ptosis and jowls forming. I felt relieved that finally a top surgeon understood how I was feeling and didn't immediately turn me away dismissing me because of my age saying I was too young etc. I knew straight away that he was the surgeon for me. A strong gut feeling and I trusted him 100%. I knew of his natural looking results from his previous work with other patients. He is truly an artist and has very fine attention to detail in his work. - Just look at his analysis of Renee Zellweger video on YouTube and you’ll understand what I mean. The morning of the surgery I was very excited and a bit nervous. I met with Will, the Anesthetist who was very nice and reassuring and put my mind at ease about the local anesthetic and sedation. I was not nervous about what Julian would do to my face as I trusted him and his work but more the fact of the anesthetic and sedation - losing control. I have had anesthetic in the past and been very sick afterwards throwing up numerous times, but Will assured me that this would not happen as they used sedation and local anesthetic instead of General Anesthetic. I remember going into the theatre and seeing Julian, Joanna the other doctor who was very reassuring and Will. Kelly the nurse who was looking after me was also there and I remember they had a big picture of me on a screen, next to another picture of me when I was 29 years old for comparison. The last thing I remember was holding the nurse Kelly’s hand and feeling nervous about the sedation and losing control. She was very kind, holding my hand and reassuring me throughout and I don’t remember anything else! I was asleep throughout the whole surgery and then remembered waking up at the end. I had no sickness and no grogginess at all! I stayed the night at 23 Harley Street and I would say the first night was the most uncomfortable and a bit of pain but was given painkillers etc…I had a lovely night nurse looking after me who was on hand the whole time and gave me the medicine. From then on, the healing took place and as the days went on I could start to see the results. I would say from week 2.5, I was able to go out in public, I did still look swollen but by then it had gone down significantly. At 3 weeks I was confident to carry on with activities, go into the office, and continue my social life. I had regular follow ups at 23 Harley Street and was very well taken care of by Dr Julian De Silva and his team. I was offered complimentary red light therapy and oxygen therapy which helped the swelling and bruising and was relaxing at the same time. It is just over 3 months now since I had the operation and I am delighted with the results! It looks so natural and fresh. I still look like me but a fresher younger version of me that no longer looks tired. My jowls have completely gone and I have my v shape jawline back. Dr Julian told me that the skin had dropped 2.5 centimeters and that is how much he had to lift it up again, which is a lot for someone my age and testament to the hyper mobility. I can now wear eyeshadow without it creasing and my eyebrows are back to where they were pre ptosis! My confidence has come back and I am feeling so much more positive about how I look! I put off going on a dating site pre surgery because I just felt I looked so tired and my confidence was low but now post surgery, I have had the courage and confidence to put myself out there. My experience with Dr Julian De Silva and his team was second to none - No other surgeon could have done better work on me. What I like about Dr Julian De Silva is his humbleness and integrity. He is also a true artist - He will look at the symmetry of your face and treat it like a work of art - this goes beyond just surgery, this is artistry. From my results, especially around my eyes I can see how intricate and finely done with attention to detail he has done everything which makes for such natural results. The whole experience from the first consultation right through to the 3 month mark with several visits in between has been so enjoyable for me. I truly loved the whole experience of my trips up to 23 Harley Street. He has a wonderful team who make you feel at home and comfortable straight away, they are very accommodating and always on hand with any questions or concerns you may have. It’s a lovely energy there. I have had consultations with other top surgeons in London and their teams over the last few years but none have been on this level. This review is long overdue as I had my rhinoplasty back in November 2022! Words honestly cannot express how incredible Dr. De Silva is!! Rhinoplasty is something I was thinking about on and off since I was about 15 or 16 years old, and when I turned 24 I decided it was something that I wanted to go through with. I've never wanted a pointy tip nose, or I guess what others would describe as an "Instagram influencer nose" - I simply wanted my nose to be straight, and natural looking. Many things did keep putting me off getting a nose job, mainly the pictures I would see of others online who visited surgeons in places like Turkey and looked severely bruised. That's why I really did my research, read reviews, and ultimately decided I would visit De De Silva. After my consultation with him, I felt that he was the right choice, and I can happily now say that I was right! Here are a few things that I was nervous about and how they went: 1. Procedure - Didn't feel a thing! I knocked out pretty quickly and woke up feeling fine 2. Post-Procedure pain - I didn't experience any pain, at all! 3. Bruising - I had slight yellow bruising under my eyes for about 3 days, but other than that I looked the exact same; no swelling! Also, although my nose was quite swollen after I had the cast taken off (Remember this, as it is a little scary to see your nose swollen at first - it DOES go down. Just like when you fall over and injure your knee, it can swell up, but it goes down again eventually. Your nose needs time to heal because it's experiencing nose trauma) the swelling went down majorly, pretty quickly too. I am so much more confident and happy with how my nose looks now - even little things like tying my hair up seemed daunting before as I didn't want to draw too much attention to my nose; and now I almost always tie it up. The best thing is, my nose fits so well with my face, it looks like it's more in harmony with my other features, whereas before I feel like you couldn't notice my other features as much because my nose drew the most attention. It's straight, completely natural looking and I'm in love with it!! Rhinoplasty is a big decision to make, so if it is something you've been thinking about for a long time, I would really recommend at least visiting Dr. De Silva for a consultation. I had an amazing ethnic (South Asian) rhinoplasty result from Dr. Julian De Silva. I had my surgery in May 2019 and wanted to wait around 1-2 years before posting my review so I could give a completely honest review for others who are looking for a good surgeon. Prior to my surgery, I had a very large and crooked nose. I also have very thick skin so my expectations were not very high before my surgery and I was not expecting to achieve anything close to my dream nose. I explained in detail what I was hoping to achieve and Dr. De Silva was 100% honest with his responses and what could realistically be achieved given I have very thick skin. This reassured me that he was not making any false promises and was giving me realistic explanations of what I could expect. I booked my surgery for 7 weeks after my first consultation and the entire process went very smoothly. I was sedated using twilight anesthesia and did not experience any pain whatsoever during the surgery or after and did not regain consciousness until being awoken by the nurses. I believe the surgery was around 90 minutes in total. The entire process was SO smooth and I experienced no pain at all. The cast was then on for exactly a week (7 days) and I experienced some mild discomfort due to having a stuffy nose but nothing worse than a normal seasonal cold. I have also prescribed some medication to help with this which I didn’t actually feel I needed but took anyway. Once the cast was off - I won’t lie - I didn’t even recognize myself which was quite distressing at first. In my eyes, I looked like a completely different person and I felt distressed to think that I didn’t look like myself anymore and everyone would ask me what I have done !! I immediately regretted my decision and was in tears thinking there’s no way I could even undo the surgery. However Dr. De Silva and the practice manager Jennifer reassured me that this was far from the actual result and that my nose would look different over the next few weeks/months to come - which is true (I just wish I had believed them so I could’ve saved myself weeks of distress). I guess nothing can prepare you for that moment! For the first few weeks after my cast removal I was really down and unhappy with the way that my nose looked - largely because of the swelling which I kept thinking was a part of the permanent result. I felt that I looked completely different and I really regretted my decision. My family members kept reassuring me that everything would be fine but it was really upsetting to think that no one would recognize me. However, by week 4-5, I was slowly starting to notice a decrease in swelling and could start to appreciate the result. Around this time I started to visit other family members and friends (who weren’t aware of the fact I was having a rhinoplasty) and they didn’t even notice any change! Nobody to this day - 18 months later - has mentioned that my nose or face looks different (and I have VERY blunt/honest family members/friends who would openly tell me if they noticed something). But I can see a MASSIVE (positive) difference. Dr. De Silva has refined my previously bulbous nose tip and smoothed the bumps on the sides of my nose. The side profile of my nose is also really nice. Whilst I don’t have a perfect nose by Western standards, Dr. De Silva has given me a very nice natural result which is harmonious with the rest of my features but makes my nose look a lot smaller than before and much more feminine. My nose is now in balance with the rest of my features and does not stand out in any way on my face. It looks nice from pretty much all angles and does not appear as wide/bulbous when I smile as it did before. Whereas before I probably took 1 selfie in a blue moon as I hated the way my nose looked, my camera roll is now literally full of thousands of selfies/photos because I am so happy with the way it looks! It sounds vain but we live in a society obsessed with appearances and in which we are often judged because of them as well. I had my doubts before deciding to get a rhinoplasty but I am SO glad that I did and trusted Dr. De Silva as this result is far better than what I was expecting. My confidence has increased so much and other people have noticed this and said that I seem much happier. I am now no longer afraid to try new hairstyles, try different makeup styles, etc as before I was afraid that certain things wouldn’t look good on me and would emphasize my nose - sounds silly but I’m sure we all do/think something like this. To all those who are still reading - I honestly do recommend Dr. De Silva with an ethnic rhinoplasty as he has completely impressed me with his skill and given me the perfect nose for my face! I can’t thank him enough! Overall, advice to anyone reading is that if you are seriously insecure about something to the point where it is affecting your day-to-day life and you want to change it - look into your options and don’t be afraid to do it! Hi everyone - just wanted to post here in case it helps anyone as I recently had a rhinoplasty with this doctor. In terms of the consultation process everything ran smoothly (especially as this is a difficult time with COVID etc). The Dr was really professional and helpful and explained fully what result was achievable. I had a very slight bump at the top of the bridge that I wanted to correct, and also wanted the top to be thinned out ever so slightly. The day of the procedure ran fairly smoothly - everything was on time. I was a bit disappointed that I didn't have a bed to recover in and had to recover in a chair instead - I said at the time I thought this was odd but was told it was due to the sedation / it's better to recover sitting up following surgery. I personally didn't think the nursing aftercare was that good in terms of bedside manner - I can only compare this with the NHS where I've had surgery before and they were a lot more caring from my point of view, which I did feedback to the clinic. In terms of the most important bit - the result - I am now 5-6 weeks post op. After the first two weeks I was really, really pleased with the result. It was exactly what I had asked for and the bump which I had hated for so long was gone, I no longer felt bad every time I looked in the mirror or saw myself from an angle. I was also really pleased with the front-on angle as initially I was worried it would be thinned out too much and not suit my face, but it does! After 3-4 weeks though I did develop a slight bump in the middle of my nose - I am quite worried about this but I'm told this is completely normal and just due to the reduction in swelling so I am trying to just trust the process and hope this goes back down in the next few weeks. I wanted to mention my race to help and encourage people of colour who might, understandably be hesitatn. Black people tend to avoid this kind of procedure. We don't trust surgeons with our faces and we scar easily and visibly. Dr De Silva is very aware of this and gives great and specific guidance. Whatever your race or ethnicity I can recommend this wonderful calm man and his lovely staff. The clinic is gorgeous and clean and welcoming. Nine months ago I had eye laser surgery elsewhere. This was also amazing I and threw away my glasses that day.... and suddenly my baggy undereyes were on display. So I started researching something I never thought I'd ever do: Cosmetic Surgery. Dr De Silva's reviews were amazing. And so was my consultation. He totally understood my needs and calmly laid out what could be done and how. I had my undereye bags seen to using fat replacement and little surgical lift to the skin. I was a little concerned about the potetial scar along my lower lids. But it looks AMAZING. My friends are eagle-eyed regarding plastic surgery and not one of them has noticed a thing. The one bestie who knows at the moment is an artist and has analytical eye. He is amazed by the results. It's not cheap. But you cannot go cheap on your face. This has been worth every single penny and the stunningly natural results are life-changing for me. This and the laser surgery have been the best double decision of my life. The lion's share of the bruising takes about two to three weeks to go down and then there is a bit that really only you can see that takes another month or two, easily dealt with make-up if you use it. (I don't use any and it's fine.) The residue of bruising is actually handy in my opinion, because, though the bags are immediately gone, your eyes do initially look a teensy bit tired in a different way and as the dark areas fade the compliments start coming - and mounting up - without anyone noticing exactly why or what's happening as they attribute your brightening face to your healthy life and diet and rest. I'm almost three months in now and VERY happy with what's happening. I'm told it will look even better in a year. Really? It looks so great now. Five stars? Can I please give ten? Ten stars! Dr Julian De Silva is an absolutely incredible surgeon with the most wonderful bedside manner. His whole team couldn't have been more lovely and helpful throughout the whole process, with no question too small or silly to ask. From the very first consultation, right through to the operation and the follow up appointments, it was such a smooth and lovely experience. I was very nervous about making sure the result was natural and would suit my face, but I didn't need to worry because Dr De Silva is an absolute artist and I couldn't be happier with my results even only 2 months after surgery. So much so that most people haven't even noticed i've had anything done because the swelling is so minimal and the results just suit my face so much. I was quite worried about having the sedation but again I needn't have worried because that was actually the easiest and nicest part of the whole procedure. The anaesthetist was so lovely and put my mind at ease from the moment we met and then going into the operation I was asleep quicker than I had expected and didn't know anything until it was over. The follow up calls from the nurses were really helpful and Dr De Silva was on call if I needed anything during the recovery process. If I could give more than 5 stars I would and I totally recommend booking a consultation with Dr De Silva and his team.
The procedure that I had with Dr De Silva went very well and I am happy with the results. Dr De Silva and his team spent a lot of time with me: they were good listeners and their answers gave me a lot of reassurance. The procedure itself was relatively pain-free. Post-procedure there is some pain and swelling but it is worth it in my opinion. I recommend Dr De Silva highly.
I had lower blepharoplasty surgery as I had a lot of wrinkles in my lower eyelids. I have big eyes so the wrinkles were very obvious and only being in my mid-thirty’s I felt it aged me. The surgery was only an hour long and Dr Julian and his team were all really nice and caring. I am extremely happy with the results and my eyes look so much better! The wrinkles are hardly there and I actually like how my eyes look in photos now! I definitely recommend Dr Julian and his team and I’m now considering what my next surgery will be!
The surgery I had with Dr De Silva was a second revision lower blepharoplasty. I had previously had a lower blepharoplasty and revision surgery with another surgeon which was a disaster and I was left with my eyes looking unnatural. I searched the internet exhaustively, desperate to find a top eye surgeon and I came across Dr De Silva. After reading reviews and the fact he did facial surgery I chose him. I am very thankful I did. I went for my consultation and Dr De Silva made me feel very positive and calm after being very nervous and apprehensive about the risk of more surgery. He was very honest about the challenge that faced him but I felt he came across confident he could improve my eyes. I somehow just knew I had found the right surgeon, I went ahead with my surgery which was absolutely pain free and I was very happy with the results. I could see myself again and the smile was back in my eyes ! I appreciate the skill it must have taken to achieve the result I got and I will be forever grateful. There is no age limit for having a rhinoplasty, what is most important is your general health. I have completed rhinoplasty for patients in their 60s and 70s, safety is paramount, and a thorough evaluation of all underlying health conditions is important. There are physiological changes in the nose that occur as we age, including changes of the cartilage and a decrease in skin elasticity that can affect the long-term results. General anaesthesia presents another risk in older patients. In the UK, we perform rhinoplasty under sedation anaesthesia that avoids these issues. Local anaesthesia with sedation is a safer option, known as “twilight anaesthesia” or “conscious sedation”, it is more commonly used in the US. I recommend you consult with an experienced facial cosmetic and plastic surgeon who specialises in rhinoplasty, they can make a full assessment of the best options for your needs.
Hi Littlemi, Most changes in the area of the neck are a combination of genetics (inherited from your parents) and some ageing. As you get older the looseness in this area is likely to increase as the natural elasticity in the skin reduces. While non-surgical treatments, including hyaluronic acid fillers may be effective in treating early soft tissue changes in the neck they are unlikely to make a significant difference to you. Similarly non-surgical devices, work more on tightening loose skin and will have limited effect. For a natural-looking rejuvenation that corrects skin laxity, fullness under your neck it is likely that you will need a combined facelift and neck lift. Often there is some deep soft tissues (fat, sometimes salivary glands) that have given rise to the fullness in your neck, this can be improved with surgery and give you a better neck profile that even when you were 20! I recommend you consult with an experienced facial cosmetic and plastic surgeon who can make a full assessment of the best options for your needs.
Hi es113, With a short chin, the optimal treatment is quite dependent on the overall size, shape and relative balance of your chin with your face. It is difficult without photographs, however a measured approach can be taken. Fillers can be used to give a temporary improvement and will make a difference, however if very small, substantial filler may be required and only gives a temporary effect. A chin implant is an augmentation and lengthening procedure that is helpful for patients who have small chin. It provides a better balance to the facial features by subtly altering the underlying structure of the jaw. For some patients other more invasive surgery may be required such as genioplasty. A reasonably straightforward surgical procedure, the chin implant is tailor-made to suit each individual and is shaped to provide a natural-looking jawline. It is a common technique as it provides a permanent solution, unlike using fillers to augment the chin which only lasts for 6-12 months. I would recommend you schedule an appointment with an experienced facial cosmetic surgeon who will be able to examine you and suggest whether a chin implant would be the most suitable solution to your problem.
Hi Taylor With your eyes, there is minor asymmetry and there is not any significant ageing. Minor asymmetry is so common in this area, and in a way is relatively normal. Even the most beautiful models, actors etc have some asymmetry, however this is often touched up in professional photo shoots. It is natural for there to be subtle changes in the eye area as we age that result in asymmetry. Although surgery or non-surgical techniques can be used if there is marked asymmetry, if relatively minor it is sometimes best left alone. Upper blepharoplasty is eyelid surgery to correct asymmetry caused by drooping or puffiness of the upper lid, whereas brow lifts (forehead lifts) are recommended for patients who have sagging or low eyebrows. You dont need this. Filler injections may also be used in the upper eyelid. They provide an intricate treatment to tighten skin, reducing droopiness and overhang. They are only a temporary measure, lasting 6-12 months. It is difficult to make a full assessment from a single photo however tweaks with non-surgical treatments can sometimes be helpful. Dear RealBeaky, Without photos, it is difficult to say which procedure you would benefit from the most. That said, it is definitely possible to perform a second facelift if you feel your personal signs of aging have returned, and there are more advance techniques to enhance results of facelift surgery that were not available decades ago (e.g. if volume loss enhancement with fat transfer or use of facial implants can enhance the result) I recommend you book an initial consultation with a facial plastic surgeon who has experience with revision facelifts.
