Do I need a full rhino or could I do a tip plasty/alarplasty? (Photos)

Question

I tried to mockup the photos as best I could. It’s not exact, but I have a large bulbous nose, and I feel like it fits my face but it’s just too large in the tip area and the width of the nostrils. I’d like a slightly more defined bridge and tip, and to bring in the nostrils.

Your nose is wide at the bridge as well as the tip.  You will need a full rhinoplasty to take care of bone and cartilage of the full length of the nose.

Hello and thank you for your question. Based on your photograph, you may benefit from a rhinoplasty. A surgeon can accomplish this by trimming, suturing, and reshaping the cartilage in your lower nose. Fascial grafts may also be used to help improve tip refinement. Make sure you are going to a surgeon with hundreds of long-term before and after pictures on their website with standardized lighting and standardized backgrounds. With good surgical technique, you could have an outstanding result. I recommend that you seek consultation with a qualified board-certified rhinoplasty surgeon who can evaluate you in person.

A rhinoplasty with an experienced American Board of Plastic Surgery certified plastic surgeon is likely your best option to achieve this goal.  You would likely need a cartilage rib graft to provide adequate cartilage graft to increase the tip projection, narrow the nose, and create a higher dorsal profile. This is all dependent on a proper history/physical exam, CT scan, ultrasound, and an extensive discussion regading goals. I also use a 3D simulation to help show what is possible surgically. From the imaging presented, it appears that you've had just an alar-plasty imaging performed  which accomplishes narrowing  wide nostrils. You may have thick skin in the tip of your nose, which is gonna impede any refinement in that area. If you're looking to narrow the bridge line, then a full rhinoplasty is required with osteotomies. A full set of pictures from all angles would be required to make that determination.
