I had advancement of the lower jaw, combined with a sliding genioplasty when I was 16 to correct an undeveloped jaw/tmj/overbite. While the surgery resulted in some noticeable improvements, there are aspects I am still unhappy with. A) the first of these being the hooked shape of my chin from the side profile- What is causing this/how can it be it be corrected to a more smooth/straight chin shape? B) My midface is elongated- how to mitigate? Upper jaw surgery, lip lift, cheek implants, etc?

Thank you for posting this. Your face is very well proportioned. There is a rule of lower facial thirds which says that in an attractive face, the distance between the nose and the mouth is half of that between the mouth and the chin. I would not recommend any surgery.
In regards to making a quality assessment, and Neurologist is probably the ideal person.
