Your pictures are very striking, you have grade 3 gynecomastia which is the most severe form. It has a large amount of excess breast tissue and a very large amount of excess skin, which you obviously have based on the pictures. This is correctable with a male breast reduction. The goal of a male breast reduction is to remove all of the excess breast, tissue and skin and leave a flat contour chest. This difference from a female breast reduction, where the goal is to create a mound of breast tissue that has projection. The incision for a male breast reduction is usually an inverted tea, also known as as an anchor. Additionally, liposuction helps to define and contour the chest as well.

Thank you for your question. You have a significant amount of excess breast tissue with loose skin. You would benefit what is called a modified mastopexy, aka gynecomastia surgery with skin excision. The scars would be a transverse scar across the chest, and the nipple areola complex would be maintained on a thinned pedicle of tissue and the areola would be made smaller. Based on your pictures in my opinion, this represents a grade 4 gynecomastia that is best and only treated with subcutaneous mastectomy and free nipple grafting. Positioning the scar, not in the middle of the chest, but where the natural crease that demarcates the chest from the upper abdomen is where the final issues should rest. Resizing the nipple and placement in the nipple in a more lateral position as a free net will top off this procedure.
