Hi 1travelbug, thank you for posting your question and pictures here!please take my answer as a personal opinion and not as medical advice since I am not privy to your medical history and I have not conducted a medical exam on you. you do have a great bone structure which helps as you already have well defined high cheeks and a nice jawline. Unfortunately with age you lose some volume in the face mainly around the cheek area and the facial tissues tend to descend. just purely based on your pictures a comprehensive approach will give you the best results , I would address all the issues for the best results but you don't have to , you can be as aggressive with the management as you want though the results might not be as dramatic if you opt for a less extensive solution. - for the volume loss component I understand that you have tried fillers and sculptra in the past but there are other options available , ideally fat grafting is the best option , it looks natural, it's your own tissue and it also improves the skin quality , if you don't have fat to harvest , you can discuss with your surgeon other options like fat matrix or cadaveric fat such as renuva. - skin quality , besides having a good skin regimen from retinols and other topical skin solutions you will likely benefit from a resurfacing procedure to improve on the fine lines this can range from chemical peels , to lasers ( such as Co2 lasers) to dermabrasion , once your surgeon examines you he/she will be able to recommend the best modality. - tissue droop/ptosis : this is best addressed with a facelift, possibly a ip lift as well , this will reposition the tissues back into a more youthful position , this will actually help a little with the volume issue as the droopy tissue that is no longer filling the desired areas will be lifted back to it's correct position , this will also improve the deep rhytids/wrinkles . Thank you for posting your question and pictures here! please take the following as an opinion and not as medical advice, for medical advice I would recommend that you consult with your local plastic surgeon so that they can do a proper history and physical exam. 
to discuss lack of lower eyelid or skeletal support a lateral profile view would be helpful. That being said you bring up a couple of other issues.
- negative canthal tilt , from the pictures the corner of your eye laterally points more downward , and that seems to be bothering you , this can be addressed with a canthopexy or canthoplasty but that being said it will change the way your eyes look and might make them appear slightly smaller .
- you also mentioned that you feel that lifing up your cheeks gives you a more desirable look , this can be from a number of reasons , lack of skeletal support is one of them which can be corrected with an implant but i cannot assess that based on the frontal pictures provided , other possible solutions if the hollowness is the result of lacking volume then fat grafting in the area can help improve it , and if that hollowness resulted from your cheeks drooping then a midface lift can help with that.
What ever the solution is please keep in mid facial surgeries in general can change the way you look permanently and each surgery carries it's own set of risks and benefits so study your options and reason for surgery well before venturing down that road.

Hi I’m 25M and I’m curious what kind of procedure could help with my lower eyelids. My eyes seem to have some lower eyelid droopiness and a bit of sclera show. Also the transition from my lower lid to cheek isn’t very smooth, causing shadowing and a tired look. I have naturally large eyes and I think the problems might come from a lack of orbital support and or lower lid laxity. When I push up on my cheek very slightly, it gives me a more desired look. I have included some photos of my eyes (first 2), as well as an AI generated version of what would be more desirable (ignore the eye color change). I’m curious if this result is realistic or a bit of a stretch. Thanks!

My answer will be based on the limited information provided so please take it as an opinion rather than medical advice , for a medical advice please consult with your local plastic surgeon or occuloplastic surgeon. For that diagnosis to be made a lot of parameters are measured and examined chief amongst them is probably the marginal reflex distance which is essentially the distance from the margin of the lid to the light reflex in the pupil , if the distance is short then it might assist in diagnosing ptosis , other factors include levator excursion..etc that are usually examined by your surgeon , additionally your surgeon might want to rule out other pathologies such as myasthenia gravis which is an autoimmune disease that causes muscle weakness at the end of the day which can initially be demonstrated by ptosis that fluctuates throughout the day ( not present in the morning but becomes prominent in the evening). based purely on your picture it does appear that you might have ptosis but again that is just an opinion based on a singular image.

As you can see in the photo, my upper eyelids are visible But when I look upward, the upper eyelid elevates and becomes much less noticeable. That’s why I’m wondering whether this is true ptosis Could this be related to muscle function, excess skin, eyebrow position, loss of brow fat volume, or something I'm not aware of?

Thank you for posting your question here! I am not sure what stitches the posts were talking about but i am going to weigh in on a couple of possible scenarios.
- fat repositioning with a lower blepharoplasty: In certain individuals with hollowness under their eyes when they undergo a lower blepharoplasty their surgeon might reposition the lower fat pads after freeing them rather than remove them to help fill that hollowness. For the first few days you want to fixate those fat pads in their new position so that they would stick and heal there as opposed to migrating and this can be done in a number of ways including possible external stitches. When those are done usually bolsters are used to offload tension from the skin and decrease scarring and they are usually removed within the first 7 days as not to leave marks , but they sometimes leave temporary marks due to the swelling around them.
- The other possibility could be the hemostatic net sutures , it is a way of avoiding a drain but at the same time decrease the chances of a hematoma or a seroma from forming , the surgeon after doing a facelift which can also be coupled with fat grafting might place a running external suture to obliterate the dead space created by the surgery so less blood or fluid accumulates, those sutures are usually removed within 72 hrs and usually don't leave permanent marks , but might leave temporary ones.
usually for just fat grafting either no stitches are placed or a single small stitch is placed at the entry site which is removed within 7 days and doesn't usually leave a scar. That being said people heal differently and some might be an exception to the above mentioned scenarios.

I've come across patient reviews that mention stitches in the face after facial fat transfer that result in scar tissue. Is it common to put stitches after the procedure? My understanding is that only very small incisions are made. Wouldn't they heal on their own?

It is very common to perform areolar reduction after placement of breast implants , and you don't have to exchange them if you do not have any issues with them. your surgeon does not have to access the implant pocket when doing an areolar reduction so the implants will remain isolated and safe. Most surgeons will place a permanent internal suture though around the areola to help prevent future spreading of the areola.

Thank you for posting your question and pictures here ! First of all Congratulations on your weight loss !, 100lbs is impressive . Unfortunately skin redundancy is very common in this situation but once addressed it will make you feel a lot better. That being said the issue you are presenting is very complex and multifaceted . 
for optimal results you will need to address both the skin redundancy and the volume loss. restoring volume is a complex issue in your situation since you don't have much fat and thus a BBL is a little tricky , you can always opt for cadaveric fat like alloclae but you might need multiple sessions and it might turn out to be expensive . Auto-augmentation can be an option that is done in conjunction with a lift but might not give you enough volume you might need to couple that with alloclae BBL etc . a buttock implant could be a shortcut and might give you the most dramatic outcome but it does have a high complication rate so you have to do your research before venturing into it since implants in general are not life long devices.
As for the skin redundancy , if it was mild to moderate then I would have suggested tring an energy device if you want to avoid scars but in your case you have a lot of redundancy that this will almost definitely not be sufficient . you also seem to have a lot of laxity at the superior aspect of your buttock that a thong lift might not be able to address , though you might have to couple the superior buttock lift with an inferior buttock tuck to also address the laxity in the posterior thigh and inferior buttock.
Those are only observations though based purely on the pictures provided , for an in depth medical advice I would recommend consulting with a plastic surgeon in person so that they can come up with a plan that works for you .

Hello! I had gastric bypass a few years ago and lost nearly 100 lbs. I’m now very thin. Around 105 lbs at 5’7 1/2”. However, I now have severe ptosis of the buttocks and posterior, medial, and lateral thigh region that I find very embarrassing. I want to mitigate scars as much as possible. What type of butt or thigh lift would best fix this problem? Could I do a Miami thong lift combined with inferior buttocks tuck or would a full superior butt lift be required? I also want to add projection to my butt as it is completely vertical now. I didn’t have much projection before the weight loss either so gaining weight will not help. I have nearly zero loose skin on my abdomen as you can see from the photos. I’m unsure if any plastic surgeon would attempt a bbl on someone’s size or if butt implants or auto augmentation are my only options. Also, can they be performed at the same time as the lift? Thank you!

There are different ways of dealing with rippling including fat grafting , fat grafting can be done a year after placement to help thicken the tissues above the implant and to better camouflage it but that being said I would discuss the issue with your surgeon as to address the root cause of the issue .
in certain situations it might be appropriate to change the type of implant to something that has less of a rippling issue , or to change the placement of the implant if it is above the muscle placing it under etc ... so a discussion with your surgeon might be helpful in making that decision .

I would say that for your middle section and for the optimal results I would consider at least lipo 360 with an extended abdominoplasty/ tummy tuck , it is hard to tell how much laxity you have in your buttocks vs cellulite , but if you have a lot of laxity there then you might want to consider adding a lower body lift , but if it is just cellulite then an extended abdominoplasty can take case of the love handles that you see in the back and give you more of an hourglass appearance, I would definitely add liposuction as adding it will help get you a better shape and makes your end result less boxy and more hour glass and when done right and with good judgement it can be coupled with the abdominoplasty surgery safely. Choosing a breast implant is a complex process , and you have to look at it in a 3D configuration , for an example for the same volume ( amount of ccs) a higher profile will give you more projection but will be narrower thus will have a smaller base thus less upper pole filling.
The main way to determine the best volume for you is using the breast width and matching that with an appropriate implant . you can always consider using the external sizers to get a better idea or if your surgeon has a 3 D imaging device such as the vectra device they can use that to give you a better idea. In any case I would encourage you not to goo too big just to avoid a lift as that will increase your chances of a possible complication in the future. If you need a lift then go for a lift. Hi i am getting a breast augmentation in december and i am very torn on what profile is going to achieve the look i want. My surgeon operates with under the muscle, and i know i will be doing silicone. I am debating between 350cc and 375cc (and also between moderate plus versus high profile. i have lost a lot of breast volume after breast feeding two children and my skin is stretched as i got up to a large d cup with my first baby and now have returned to 32 band size and probably a b, hard to tell since it’s mostly just loose skin. I want the results to give me more volume st the top of my breasts but i do not want a large gap in between my boobs, also ideally want my nipples to point out as straight as possible, any advice would be greatly appreciated.

People with excess skin just above their belly button are good candidates for a reverse tummy tuck . In a reverse tummy tuck the incisions are made in the breast fold to lift up the skin and tighten it . That being said if your issue is more of fibrosis then the skin might not be mobile enough to do that . In that case you might need a full on tummy tuck to elevate the skin and re- drape it , if that is the case then you might have an additional small vertical scar as you might not have enough skin laxity to pull the opening were the belly button was down to the incision at the bottom .
I would recommend consulting with a plastic surgeon in person so that they can examine you and check your skin mobility and offer you the best surgery for your situation . best of luck!

I want to feel confident again. I’ve had two rounds of a bbl. One in 2018 other in 2021. At this point I believe my buttocks is too big for my frame and unhappy with my tummy after having my son in 2023. I do Believe I have Fibrosis as well. I feel Like most of my excess skin is above my belly button. Any advice?
