My answer should not be considered as medical advice since I am not privy to your medical information nor have I examined you.
I would say that for your middle section and for the optimal results I would consider at least lipo 360 with an extended abdominoplasty/ tummy tuck , it is hard to tell how much laxity you have in your buttocks vs cellulite , but if you have a lot of laxity there then you might want to consider adding a lower body lift , but if it is just cellulite then an extended abdominoplasty can take case of the love handles that you see in the back and give you more of an hourglass appearance, I would definitely add liposuction as adding it will help get you a better shape and makes your end result less boxy and more hour glass and when done right and with good judgement it can be coupled with the abdominoplasty surgery safely. Hi! After losing over 100 pounds, my skin is dripping off of me! Obviously a breast reduction and lift is in order, but what do you recommend for the midsection-—TT or LBL? And would you do no Lipo, Lipo to front only, or Lipo 360? Thanks in advance for your input!

Choosing a breast implant is a complex process , and you have to look at it in a 3D configuration , for an example for the same volume ( amount of ccs) a higher profile will give you more projection but will be narrower thus will have a smaller base thus less upper pole filling.
The main way to determine the best volume for you is using the breast width and matching that with an appropriate implant . you can always consider using the external sizers to get a better idea or if your surgeon has a 3 D imaging device such as the vectra device they can use that to give you a better idea. In any case I would encourage you not to goo too big just to avoid a lift as that will increase your chances of a possible complication in the future. If you need a lift then go for a lift. Hi i am getting a breast augmentation in december and i am very torn on what profile is going to achieve the look i want. My surgeon operates with under the muscle, and i know i will be doing silicone. I am debating between 350cc and 375cc (and also between moderate plus versus high profile. i have lost a lot of breast volume after breast feeding two children and my skin is stretched as i got up to a large d cup with my first baby and now have returned to 32 band size and probably a b, hard to tell since it’s mostly just loose skin. I want the results to give me more volume st the top of my breasts but i do not want a large gap in between my boobs, also ideally want my nipples to point out as straight as possible, any advice would be greatly appreciated.

People with excess skin just above their belly button are good candidates for a reverse tummy tuck . In a reverse tummy tuck the incisions are made in the breast fold to lift up the skin and tighten it . That being said if your issue is more of fibrosis then the skin might not be mobile enough to do that . In that case you might need a full on tummy tuck to elevate the skin and re- drape it , if that is the case then you might have an additional small vertical scar as you might not have enough skin laxity to pull the opening were the belly button was down to the incision at the bottom .
I would recommend consulting with a plastic surgeon in person so that they can examine you and check your skin mobility and offer you the best surgery for your situation . best of luck!

I want to feel confident again. I’ve had two rounds of a bbl. One in 2018 other in 2021. At this point I believe my buttocks is too big for my frame and unhappy with my tummy after having my son in 2023. I do Believe I have Fibrosis as well. I feel Like most of my excess skin is above my belly button. Any advice?

I am sorry that you have to deal with this issue , I am sure it's frustrating . The fact that you started bottoming out this early and that you had this issue twice raises the question on how good the tissue quality is and wether it can sustain an implant . There is no wrong or right answer , each surgeon will have their way of dealing with this issue. you can always consider explantation , possible wedge excision and re-enforcement of the IMF with sutures or a mesh , with potential fat grafting to the upper pole. and later down the line if you are not satisfied with the size and fullness either consider further fat grafting or placement of an even smaller implant . That being said it is hard to predict how your tissue will react to it and there is always the possibility of having this happen again.

In 2023, I had a BA and after 8 mo post op, my implants bottomed out and my areolas expanded. September 2024, I had a revision with a BL, capsulorraphy internal bra, implant exchange and areola reduction. From my BA, I had 440cc, high profile, motiva implants. I exchanged them for 385cc, high profile, mentor implants. I’m now 6 months post op and my implants have again bottomed out, although I went smaller in size, removed 200 grams of natural breast tissue and did the internal bra. My surgeon wants to do a second revision and this time stitch my muscles onto where my ribs are instead for extra support. My skin has very high elasticity so I can understand some stretching but I have zero upper pole fullness and my nipples are extremely high. Immediately after surgery I noticed the implant placement was still pretty low on my chest despite my goal wanting it to be extra high and tight because I know my body and how my skin reacts to pressure. My surgeon has advised me that she will not be able to lift my implants up a couple inches because “it’s not possible to create a new breast fold”. I do not want an internal mesh so I’m wondering if anyone can give me second opinions on my situation! Thank you

Breast augmentation on its own does not make your nipples or areolas smaller actually on the contrary the areola might stretch as the breast size increases , that being said , an areolar reduction and a nipple reduction can be added to a breast augmentation to give you the look you want. Your surgeon will likely place a permanent suture around your areola after reducing it to prevent it or the scars from stretching in the future.

I am sorry that you are not happy with your aesthetic outcome, but this can be revised and improved. without an exam it will be hard for me to accurately give you a plan , but just based of the pictures it looks like the fat around your belly button was slightly over resected and the stalk was not tacked to the underlying fascia, this leads to flattening of the belly button and leads to the scar being more obvious , that in addition the the umbilicus being made slightly larger. this can be dealt with with a number of approaches that are best discussed with a plastic surgeon that you trust who will perform an exam and decide on the best course of action. So I had belly button revision after I got a botched belly button after a tummy tuck by the same surgeon. So he only addressed a little bit of skin on the top. As you can see my belly button is awful at the moment. I am in the process of having consultations with other surgeons as I can’t live with it. A response I got from one surgeon was “That’s exactly what we’re planning to do. The sides of the abdomen are slightly raised, while the center of the belly sits lower — and that’s the main issue. We’ll add some fat to the sides to even them out, and then contour the center inward” Another surgeon said it just needs pulling inwards. Can someone please give me some input on what the best outcome will be

You don't seem to have much loose skin in your lower face, i am guessing that drooping cheeks and some of the hollowing of your lower cheeks is what is concerning to you , this tends to age people and if that is the concern then a combination of midface lift to reposition the cheeks and potentially fat grafting can give you a more youthful appearance. Im a 45 year old male. Naturally thin. In good health. And a past smoker, and sun worshipper. After a year of taking great care of myself I have decided to look into a facelift to help refresh my appearence even if only modestly. My main concern is the area of laxity all around the mouth. Ideally id hope to see some modest improvmemt through a mini or cheek lift. If I press two fingers on my temples by my ears and move it up maybe 3/4 a inch it seems to give reasonble improvment in the area of the face from nose corners and mouth corners (mid?) Which is the area of concern. Though admitedly I can play around this way even from lower on the face and see improvments. I also seem to take a much better selfie laying down than standing up. Ideally there will be a slight softening of the abrupt countors of my face (example mouth area to lower cheek etc). I've spoke with one surgeon but want more input.

Thank you for posting your question here !My answer will be based on the limited information and pictures provided, for a more detailed and personalized plan please consult with your surgeon in person.Liquid rhinoplasty is essentially using filler to create an illusion of a better shaped nose, this wiill require maintenance and if your surgeon or injector is not skilled or unaware of the anatomy this can be a dangerous procedure and it has a higher risk of vascular occlusion not to mention the buildup of filler that will eventually thicken the nose tissue.If you are willing to undergo a traditional rhinoplasty then I think that will be a more permanent and better solution.I hope this helps, best of luck !

My nose has always looked out of place on my face. It seems too big. From research and layman’s knowledge I think it’s my bridge and tip that cause this. I was hoping liquid rhinoplasty might help. Could liquid rhinoplasty help or is traditional the only option?

I am sorry that you are not happy with your post surgical outcome, but you seem to have realistic and reasonable expectations as to what can potentially be done to corrected the issue. There are many ways to build up the dorsum of the nose , including harvesting rib cartilage or obtaining a piece of fascia ....etc , your surgeon will decide on the best option for you after examining you . The bridge of my nose has been over resected and scooped in my previous rhinoplasty and I am hoping a revision can fix this . Can the bridge of the nose be build to make it bigger and improve the shape. In the photos I used scar gel to build the nose bridge into my desired shape to give an idea of what I’m looking for. Does anyone who specialize in revisions knows if this can be surgicaly done and are my expectations realistic in the photos ?
