Thanks for your question and your concern. Chemotherapy agents and steroids have been used with some moderate success. I recommend surgical removal. The incision is hidden inside the smile line- no toxins or agents hurting your body. It appears that the filler has either been injected too superficially or that you have acquired a keloid/granuloma.
If steroid injections have not helped then it may have to be excised.  This will leave you with a scar.
A good surgeon could hide it in the nasolabial fold, but it will most likely remain visible.  If you are susceptible to keloids you would need to remember that this can then also cause a keloid.

You need to see a filler expert.  Possible treatment options include injections of Kenalog or 5-fluorouracil.  It will likely take a series of treatments for best results.  Other options might include lasers or, as a last resort, surgical excision.  Find the most experienced doctor in your area.Regards,Dr. Ort
