Question

I am a 26F and I've always struggled with a heavy lower face/jowl area. I have a pretty flat face with low cheekbones and the only structure I have is my jaw. However, I have this excess fat/skin in the bottom half of my face that is quite prominent. I have tied masseter box, cheek filler, Morpheus, weight loss to reduce this part of my face but no luck. If I take my fingers and pull the skin my skin back, I see my desired results. I really want to discuss a more permanent solution.

You have a couple of options. Buccal fat removal would help to reduce some of the fullness in your lower cheeks and take some of the weight off your jowls. However, your chin is short and recessed, which does not provide adequate support for the tissue and skin in your lower face, which is what creates that pouch. You are an excellent candidate for a genioplasty. This procedure would move your chin forward and down, bringing more balance to your profile, elongating your jawline, and creating more definition/slimming down the lower cheek area that bothers you. You've tried some good options. There are a few things to consider. These include (1) Buccal fat removal - could likely help, (2) accutite - a stronger version of energy skin tightening which can really target the area, (3) a chin implant - bringing your chin forward will help somewhat, (4) bringing the skin back slightly.
