Unfortunately you cannot have all of buccal fat removed. It expands up to your temple, towards the back of your ear and down to the jaw. Buccal fat provides cushion for structures and help muscles to glide when they contract. That being said, most people do have some extra fat in the lower cheeks that can be removed to help create more contour in the face. As a surgeon, it is our job to help you get the best results possible, while continuing safe practices. Surgeons do have influence when it comes down to how much they are willing to remove. Unfortunately there are surgeons who will remove more than they should to achieve patient satisfaction without considering long term effects. When it comes to the surgery, a surgeon and patient should have very clear expectations as to what is possible and safe, and what is not.
