The technique I recommend is The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery.

It is possible to do a revision and reposition the mammary tissue with minimal scarring improving the overall proportion and correcting the nipples. This will create overall harmony and this can be made with an L shape incision so you won't have scars in the cleavage area. Considering that you had last surgery relatively recently, you should wait for your result to settle. Your nipples need to be closely of the same size and level. With one nipple being relatively high, this needs to be addressed by moving breast fold slightly higher, to place nipple at the center of the breast mound. Hope this helps.

You need a revision of your breast lift, especially on the right side. You need nipple reduction and scar revision to help with symmetry. The left side is also an option for scar revision, but that is up to you. Please visit with a board-certified plastic surgeon as revision breast surgery is very complex. Good luck!

Right now, you need to let your tissues heal, but micro-needling can be very helpful on the scars. Also, avoid tanning for at least 6 months.

I am glad that you were able to have a revision with your primary surgeon. At this juncture, what I would do is try to put some time between your last surgery and your next surgery. The best chance is once you hit a healed steady state to provide goal directed revisions to each breast to provide symmetry. Sometimes this can be revising scars, and removing a small amount of tissue in certain areas. Additionally, this can mean Fat grafting to the areas to improve symmetry. Not all is lost and you have some good options before you. Remember that patience is often a help in revisionary surgery. Have a conversation about your goals with a board-certified plastic surgeon. Your breasts are asymmetrical so I would say that the left breast needs a revision mastopexy to make them more symmetrical.
