You have type II ptosis, you will need Breast lift, and it will depend if you want bigger breast or not, to use breast implants or not

Your grade of ptosis is bordering Grade 2 and Grade 3. At grade 2, the nipples are below the inframammary fold but above the lower pole of the breast. At Grade 3, the nipples are sitting at the lowest pole of the breast. While your nipples are not all the way at the lowest pole, they are lower than what I would typically consider Grade 2 ptosis.
It appears you have grade II possibly III. The technique I recommend is The Bellesoma Method. This will reduce and reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. The weight of the breast is transferred to the underlying pectoralis major muscle resulting in pain relief without excessive reduction. I estimate your ptosis as grade II, bordering on III. Management of grade II/III ptosis will require true mastopexy (breast lift) and there are many options for type of lift. The other option, if this fits your desire, is breast reduction which involves elevating the nipple as part of the surgery. I hope this helps.
You will need an anchor scar pattern that can effectively remove excess skin in both vertical and horizontal vectors. In addition, you will need some tissue reduction, especially in the lateral aspects of your breasts.
Based on these photos you have grade 2 ptosis. In your case breast reduction with lift would be a good option. Breast lift only, will leave you with haevy breasts which will soon create secondary deformity. Good luck.

Dear VictoriaValencia1,my preferred lift is a lollipop lift when a periareolar won't do (ie greater than 2cm lift required). The lollipop technique was created by a Canadian plastic surgeon named Elizabeth Hall-Findley and treats the breast as a three dimensional structure instead of two dimensions (anchor - lift). Most importantly it does not require the horizontal component scars that the anchor-lift requires and breasts appear much more natural, lifted, and less boxy. Lastly, the lift lasts longer because the lollipop doesn't rely on the skin to hold the breast up, its actually breast tissue thats being brought together to support the breast and hence improves longevity. If you are considering surgery, I would suggest you consult a board-certified plastic surgeon. You have grade four and will need the full lift. A reduction would also work well for you and relieve you of the heavy weight.

Hi, thanks for your question. You may benefit from a small lift but a breast implant would more reliably provide improved upper pole fullness and firmness. I would encourage you to discuss your goals and the best way to achieve them with a plastic surgeon.
You are not standing straight for your photo, but I suspect you will need more than just the doughnut lift for your breasts. An examination and measurements are needed to be certain as to what you would need.
